<summary type="text">Effect of Lifestyle Interventions on Gestational Weight Gain Among Pregnant Women with Overweight: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Background/Objectives: Excessive gestational weight gain (GWG) among pregnant women with overweight is associated with adverse maternal and neonatal outcomes. Previous reviews have often combined women with overweight and obesity, limiting overweight-specific evidence. This systematic review and meta-analysis evaluated the effects of lifestyle interventions on GWG among pregnant women with pre-pregnancy overweight. Methods: PubMed, Embase, Scopus, and CINAHL were searched for randomized controlled trials (RCTs) published between January 2015 and December 2025. Eligible studies included pregnant women aged ≥ 18 years with pre-pregnancy overweight who received lifestyle interventions incorporating dietary and physical activity (PA) components compared with usual antenatal care. Risk of bias was assessed using the Joanna Briggs Institute (JBI) Critical Appraisal Checklist for RCTs. Random-effects meta-analysis was performed to calculate pooled mean differences (MDs) with 95% confidence intervals (CIs). Results: Seventeen reports representing 15 unique RCTs involving 6256 participants were included in the systematic review, of whom 2216 were included in overweight subgroup analyses. Twelve studies contributing data from 1728 women with overweight were included in the meta-analysis. Common support strategies included behavioral support through goal setting and self-monitoring, as well as digital or remote follow-up. Lifestyle interventions were associated with a significant reduction in total GWG compared with usual antenatal care (MD = −1.44 kg; 95% CI: −2.34 to −0.55; p &lt; 0.05). Substantial heterogeneity was observed across studies (I2 = 76%). Limitations included substantial heterogeneity in intervention characteristics, limited reporting of blinding procedures, and the relatively small number of studies available for subgroup analyses. Conclusions: Lifestyle interventions incorporating dietary and PA components were associated with significant reductions in GWG among pregnant women with overweight. These findings support the integration of lifestyle interventions into antenatal care to promote appropriate GWG. Further research is needed to identify the most effective intervention components and optimize intervention implementation in this population. Registration: PROSPERO CRD42023473693.

<summary type="text">Roles and Outcomes of Advanced Practice Nurses in Thailand: A Systematic Review

Background and Objectives: Advanced practice nurses (APNs) play an increasingly important role in improving healthcare quality and outcomes. This systematic review synthesized evidence on APN roles and healthcare outcomes in Thailand. Methods: The review protocol was prospectively registered in PROSPERO (CRD420261331793). Four databases (PubMed, CINAHL, Scopus, and Thai Journals Online) were searched from inception to January 2026. Two independent reviewers performed study selection, data extraction, and methodological quality assessment using the Mixed Methods Appraisal Tool (MMAT, 2018) for mixed-methods studies and the Joanna Briggs Institute (JBI) critical appraisal checklists for quasi-experimental, analytical cross-sectional, and qualitative studies. A narrative synthesis was undertaken because of methodological heterogeneity. Results: Twenty studies were included, comprising eight quasi-experimental studies, six cross-sectional studies, four mixed-methods studies, one retrospective study, and one qualitative descriptive study. APNs performed diverse roles, including direct clinical care, care coordination, consultation, patient education, mentoring, and implementation of evidence-based practice. APN-led models of care were associated with favorable clinical outcomes, enhanced self-care and quality of life, increased patient and caregiver satisfaction, and reduced hospital length of stay and healthcare costs. Conclusions: The available evidence suggests that APN-led models of care may be associated with favorable patient, family, and health system outcomes in Thailand. However, these findings should be interpreted cautiously because the evidence was derived predominantly from non-randomized and observational studies, and the independent effects of APN practice could not always be isolated.

การวางแผนการดูแลล่วงหน้า (Advance Care Planning) มีความสำคัญอย่างยิ่งในการเพิ่มคุณภาพการดูแลระยะสุดท้าย โดยเฉพาะในผู้ป่วยที่มีภาวะเจ็บป่วยเรื้อรัง อย่างไรก็ตาม การมีส่วนร่วมของพยาบาลในกระบวนการนี้ยังคงมีความซับซ้อน การศึกษาในครั้งนี้เป็นการศึกษาเชิงทำนายมีวัตถุประสงค์เพื่อ 1) ศึกษาระดับความตั้งใจของพยาบาลวิชาชีพในการมีส่วนร่วมวางแผนการดูแลล่วงหน้าร่วมกับผู้ป่วยที่มีภาวะเจ็บป่วยเรื้อรัง และ 2) เพื่อศึกษาปัจจัยที่มีอิทธิพลต่อความตั้งใจของพยาบาลวิชาชีพในการมีส่วนร่วมวางแผนการดูแลล่วงหน้าร่วมกับผู้ป่วยที่มีภาวะเจ็บป่วยเรื้อรัง กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพปฏิบัติงานในหอผู้ป่วย อายุรกรรม ศัลยกรรม และหอผู้ป่วยพิเศษโรงพยาบาลในจังหวัดสมุทรปราการ จำนวน 233 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วย 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามความตั้งใจของพยาบาลวิชาชีพในการวางแผนการดูแลล่วงหน้า และ 3) แบบประเมินบรรยากาศองค์การ มีค่าดัชนีความตรงเชิงเนื้อหา (Content validity index: CVI) เท่ากับ 1.00 ทุกฉบับ และค่าความเที่ยงสัมประสิทธิ์แอลฟาครอนบราค (Cronbach’s Alpha Coefficient) มีค่าเท่ากับ .916 และ .973 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และการวิเคราะห์สัมประสิทธิ์ถดถอยพหุคูณแบบนำตัวแปรเข้าทั้งหมด (Enter Regression)ผลการวิจัยพบว่า กลุ่มตัวอย่างมีคะแนนเฉลี่ยความตั้งใจในการวางแผนการดูแลล่วงหน้าของพยาบาลวิชาชีพส่วนใหญ่อยู่ในระดับสูงภาพรวมอยู่ในระดับปานกลาง (Mean = 14.31, SD = 2.721) ปัจจัยทำนายประกอบด้วย ทัศนคติต่อการวางแผนการดูแลล่วงหน้า การคล้อยตามกลุ่มอ้างอิง การรับรู้ความสามารถในการควบคุม ความรู้ของพยาบาลต่อการมีส่วนร่วมในการวางแผนการดูแลล่วงหน้า ประสบการณ์การได้ยินเกี่ยวกับการวางแผนการดูแลล่วงหน้า และบรรยากาศองค์การสามารถร่วมกันทำนายความตั้งใจของพยาบาลวิชาชีพในการวางแผนการดูแลล่วงหน้าได้ร้อยละ 38.8  (Adjust R2 = .388, p&lt;.05)จากผลการศึกษาสะท้อนให้เห็นว่า จำเป็นต้องการกำหนดนโยบายและโครงสร้างการทำงานสนับสนุนให้มีส่วนร่วมการวางแผนการดูแลล่วงหน้า ร่วมกับจัดอบรมเพิ่มเติมเกี่ยวกับการวางแผนการดูแลล่วงหน้า และการสื่อสารเพื่อสร้างความมั่นใจให้แก่พยาบาลในการดำเนินกระบวนการวางแผนการดูแลล่วงหน้า; Advance Care Planning (ACP) is essential for enhancing the quality of end-of-life care, particularly in patients with chronic illness. However, nursing involvement in this process remains complex. This study aimed to: 1) describe the level of intention among professional nurses to participate in ACP for patients with chronic illnesses, and 2) identify factors influencing this intention. A predictive correlational study was conducted among 233 professional nurses working in medical, surgical, and private wards at hospitals in Samut Prakan Province, Thailand. Data  were collected using: 1) a personal data questionnaire, 2) the Nurses’ Intention to Participate in ACP Scale [NIPACP], and 3) An Organizational Climate Questionnaire [AOCQ]. The instruments yielded a Content Validity Index (CVI) of 1.00 and Cronbach’s alpha coefficients of .916 and .973, respectively. Data were analyzed using descriptive statistics and multiple regression analysis (Enter method).The majority of the participants reported a high level of intention to participate in ACP, while the overall mean score was at a moderate level (Mean = 14.31, SD = 2.721). The combination of attitude toward ACP, subjective norms, perceived behavioral control (PBC), ACP knowledge, hearing experience regarding ACP, and organizational climate significantly predicted nurses’ intention, accounting for 38.8% of the total variance  (Adjust R2 = .388, p&lt;.05).These findings emphasize the necessity of establishing clear organizational policies and structural support to facilitate ACP participation. Furthermore, providing specialized ACP training and communication workshops is essential to bolster nurses’ confidence and competence, ultimately ensuring the effective implementation of advance care planning for patients with chronic conditions.

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างสมรรถนะทางวิชาชีพพยาบาลกับความพร้อมในการ ประกอบอาชีพการพยาบาล กลุ่มตัวอย่าง คือ นักศึกษาพยาบาล คณะพยาบาลศาสตร์ มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ ชั้นปีที่ 4 จำนวน 127 คน เครื่องมือวิจัยมี 3 ส่วน ได้แก่ 1) ข้อมูลทั่วไปของนักศึกษาพยาบาล 2)แบบสอบถาม เกี่ยวกับสมรรถนะของพยาบาลวิชาชีพพยาบาลของนักศึกษาพยาบาล ชั้นปีที่ 4 3) แบบสอบถามเกี่ยวกับความพร้อม ในการประกอบอาชีพทางการพยาบาลของนักศึกษาพยาบาลชั้นปีที่ 4 ตรวจสอบความตรงเชิงเนื้อหาด้วย ผู้ทรงคุณวุฒิจำนวน 3 ท่าน ทุกข้อมีค่าความตรง (IOC) ตั้งแต่ 0.70-1.00 ทุกข้อ และมีค่าสัมประสิทธิ์แอลฟาของครอนบาค โดยรวมทั้งฉบับเท่ากับ 0.98 ผลการวิเคราะห์ค่าสัมประสิทธิ์สหสัมพันธ์ระหว่างสมรรถนะทางวิชาชีพ พยาบาลกับความพร้อมในการประกอบอาชีพการพยาบาล พบว่า สมรรถนะทางวิชาชีพพยาบาลมีความสัมพันธ์ทางบวกในระดับสูงกับความพร้อมในการประกอบอาชีพการพยาบาล (r = .76, p-value = .000) อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01; This study aimed to investigate the relationship between nursing professional competency and readiness for nursing practice among fourth-year nursing students. The sample consisted of 127 fourth-year students from the Faculty of Nursing, Huachiew Chalermprakiet University. The research instruments comprised three sections: (1) General information of nursing students, (2) A questionnaire assessing nursing professional competency among fourth-year nursing students and (3) A questionnaire assessing readiness for nursing practice among fourth-year nursing students , Content validity was evaluated by three experts, Item-Objective Congruence (IOC) values ranging from 0 . 70 to 1.00 for all items. The reliability of the instruments was confirmed using Cronbach’s alpha coefficients with an overall coefficient of 0.98. The findings indicated a strong positive correlation between nursing professional competency and readiness for nursing practice (r = .76, p-value = .000), demonstrating a statistically significant association at the 0.01 level. These results suggest that higher levels of professional competency are closely linked to enhanced r e a d i n es s f o r nursing practice among final-year nursing students.
