การพัฒนาแนวปฏิบัติการพยาบาลเพื่อสร้างเสริมสมรรถนะแห่งตนให้ผู้ใช้บริการในชุมชนที่เป็นโรคหอบหืดซ้ำ

Abstract

การศึกษานี้ มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติการพยาบาลเพื่อสร้างเสริมสมรรถนะแห่งตนให้ผู้ใช้บริการในชุมชนที่เป็นโรคหอบหืด โดยใช้รูปแบบการใช้หลักฐานเชิงประจักษ์ ของศูนย์ปฏิบัติการพยาบาลขั้นสูง ประเทศสหรัฐอเมริกาเป็นหลัก เริ่มต้นด้วยการวิเคราะห์ปัญหา จากประสบการณ์ในการปฏิบัติงาน จากนั้นค้นหาหลักฐานเชิงประจักษ์ โดยกำหนดคำสำคัญเพื่อ สืบค้นข้อมูล ได้งานวิจัยที่เกี่ยวข้องระหว่าง พ.ศ. 2535-2549 จำนวน 10 เรื่อง ประกอบด้วยงานวิจัย เชิงทดลองที่มีการสุ่มและมีกลุ่มควบคุม 2 เรื่อง งานวิจัยถึงทดลอง 4 เรื่อง งานวิจัยเชิงพรรณนา 3 เรื่อง และงานวิจัยเชิงเปรียบเทียบ 1 เรื่อง ประเมินความน่าเชื่อถือของหลักฐานข้อมูลโดยใช้เกณฑ์ ของโพลิทและเบค จากนั้นวิเคราะห์และสังเคราะห์งานวิจัยดังกล่าว จนได้แนวปฏิบัติการพยาบาล ที่ประกอบด้วย 1) การประเมินข้อมูลส่วนบุคคล ปัจจัยกระตุ้นให้เกิดอาการ ความรุนแรงของ อาการหอบ สมรรถภาพของปอดและสมรรถนะแห่งตน 2) การวินิจฉัยสมรรถนะแห่งตนของผู้ที่เป็นโรคหอบหืดซ้ำแบ่งเป็น 3 ระดับ คือ ระดับมาก ระดับปานกลาง และระดับน้อย 3) การปฏิบัติการพยาบาลของผู้ใช้บริการตามระดับการวินิจฉัยสมรรถนะที่ประเมินได้ ประกอบด้วย แนวทางการให้คำแนะนำของพยาบาล แนวทางการสร้างเสริมสมรรถนะของผู้ใช้บริการและ ครอบครัวและแนวทางการเยี่ยมบ้าน โดยเน้นให้ผู้ใช้บริการสร้างประสบการณ์ใหม่ด้วยตนเอง ร่วมกับครอบครัวจากแหล่งเรียนรู้ 4 แหล่ง คือ ความสำเร็จจากการลงมือกระทำด้วยตนเอง การ สังเกตตัวแบบอื่นในและนอกครอบครัว การชักจูงด้วยคำพูด การสนับสนุนสภาวะทางร่างกายและอารมณ์ นำแนวปฏิบัติที่สร้างขึ้นไปหาความเป็นไปได้ในการใช้จริง โดยขอความคิดเห็นจากแพทย์และพยาบาลวิชาชีพที่ปฏิบัติงานในแผนกผู้ป่วยนอก โรงพยาบาลบางปะหันและปฏิบัติงานในหน่วยบริการสุขภาพระดับปฐมภูมิ จังหวัดพระนครศรีอยุธยา รวมทั้งผู้ใช้บริการจำนวน 5 ราย เป็นระยะเวลา 2 สัปดาห์ นำข้อเสนอแนะที่ได้ทั้งหมดมาปรับโครงสร้าง องค์ประกอบและสาระจนได้แนวปฏิบัติการพยาบาลที่ชัดเจน ข้อเสนอแนะสำหรับการศึกษาครั้งนี้ ควรนำแนวปฏิบัติการพยาบาลไปใช้กับผู้ใช้บริการที่ เป็นโรคหอบหืดซ้ำในโรงพยาบาลใกล้เคียงและเผยแพร่แนวปฏิบัติไปสู่สถานบริการพยาบาลระดับ ต่างๆ เพื่อสร้างเครือข่ายการจัดการสุขภาพที่มีประสิทธิภาพ มีการติดตามเยี่ยมบ้านอย่างต่อเนื่อง เพื่อประเมินประสิทธิภาพของแนวปฏิบัติการพยาบาลในระยะยาว และประเมินความพึงพอใจของ ผู้ใช้บริการและครอบครัว ผนวกแนวปฏิบัติการพยาบาลนี้เข้าเป็นส่วนหนึ่งของการทำงานจน สามารถใช้ได้จริงอย่างกลมกลืนแบบเชื่อมโยงเครือข่ายการให้บริการของหน่วยงานในโรงพยาบาล และหน่วยบริการสุขภาพชุมชนระดับปฐมภูมิ เพื่อให้การดูแลสุขภาพผู้ใช้บริการที่เป็นโรคหอบหืดซ้ำเป็นระบบอย่างต่อเนื่องจนเพื่อการพัฒนาคุณภาพบริการในที่สุด

The purpose of this study was to develop the clinical nursing practice guideline (CNPG) for self-efficacy enhancement in recurrent asthmatic clients in community. The center for Advanced Nursing Practice Evidence-based Practice Model, U.S.A. was modified and used as a major framework of the study. The development began with problem analysis from actual nursing experience in primary and secondary health care setting. Keywords were specified for searching the relevant evidences. Then ten related literatures in 1992-2006 were selected including two experimental studies, four quasi-experimental studies, three descriptive studies and a comparison study. All relevant evidences were evaluated based on Polit & Beck.They were analyzed and synthesized in order to develop the CNPG for Self -efficacy Enhancement in Recurrent Asthmatic Clients. This CNPG consisted of 1) self-efficacy assessment such as patient demographic characteristics, trigger factors, severity levels of asthma, lungs function and self-efficacy level 2) self-efficacy of recurrent asthmatic patients diagnosis that comprised of three levels : severe, moderate and mild 3) nursing practice for clients accorded to level of self efficacy diagnosis, which consisted of guideline for health education, self-efficacy enhancement for clients and family, and home visit guideline. This CNPG was emphasized on enhancing self efficacy of clients and family participation from 4 learning sources: successful experience from himself, role model observation from in and out family, verbal persuasion, and physical and emotional support from others. It had been used try out by health care providers worked in outpatient department at Bangpahan hospital and worked in primary care unit in Ayuthaya province with five patients for two weeks. Finally, this CNPG was last improved from all of suggestions in its construct, component, and content. This study suggested that the effectiveness of this CNPG should be done in the recurrent asthmatic clients at the hospital nearby. Also, it should be distributed to other health care services in order to create health care. The continuingly follow-up home visit was suggested in order to assess the efficiency of this CNPG for a long period of time. The assessment of client and family satisfactions should be done. Furthermore, this CNPG should be the effective tool for connective health care among hospital and community in order to create systematically health network and improve quality care for asthmatic clients.
