ผลของสมุนไพรจีนป๋ายหู่เซียนตานร่วมกับการฝังเข็มต่อความแปรปรวนของอัตราการเต้นของหัวใจในกลุ่มตัวอย่างสุขภาพดี

Abstract

สมุนไพรจีนป๋ายหู่เซียนตานเป็นสมุนไพรตำรับใหม่ที่พัฒนาโดยคณะแพทย์แผนจีน มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ ประกอบด้วยสมุนไพรจีน 12 ชนิด มีสมุนไพรหลักซึ่งมีปริมาณมากกว่าสมุนไพรอื่น 3 ชนิด ซึ่งมีผลช่วยเพิ่มประสิทธิภาพการทำงานของระบบประสาท หัวใจและหลอดเลือดและระบบภูมิคุ้มกัน แต่อย่างไรก็ตาม ปัจจุบันยังไม่มีการศึกษาถึงฤทธิ์ของสมุนไพรดังกล่าวต่อการทำงานของระบบประสาทออโตโนมิก ดังนั้นการศึกษานี้ จึงมีวัตถุประสงค์เพื่อศึกษาผลของสมุนไพรตำรับป๋ายหู่เซียนตานและป๋ายหู่เซียนตานร่วมกับการฝังเข็มต่อการทำงาน ของระบบประสาทออโตโนมิกโดยประเมินจากค่าความแปรปรวนอัตราการเต้นของหัวใจ แบ่งกลุ่มตัวอย่างทั้งหมด ออกเป็น 4 กลุ่ม ได้แก่ กลุ่มฝังเข็มหลอกหรือกลุ่มควบคุม (sham) ฝังเข็มหลอกร่วมกับกินสมุนไพรจีนป๋ายหู่เซียน ตาน ฝังเข็ม และกลุ่มฝังเข็มร่วมกับกินสมุนไพรจีนป๋ายหู่เซียนตาน สำหรับกลุ่มที่ต้องกินสมุนไพรจะได้รับยาในช่วง เช้าแล้วหลังจากนั้น 4 ชั่วโมงจะถูกฝังเข็มหรือวัดคลื่นไฟฟ้าหัวใจ คลื่นไฟฟ้าหัวใจจะวัดทั้งก่อน ระหว่าง และหลัง การทดลอง ผลการศึกษาพบว่าความดันเลือดขณะหัวใจคลายตัวมีค่าลดลงภายหลังการฝังเข็มเมื่อเทียบกับก่อน ฝังเข็มแต่มีค่าเพิ่มขึ้นในกลุ่มฝังเข็มร่วมกับได้รับสมุนไพรเมื่อเทียบกับกลุ่ม sham การลดลงของความดันเลือดอาจ เนื่องมาจากการฝังเข็มมีผลลดความต้านทานของหลอดเลือดโดยการทำให้หลอดเลือดขยาย ส่วนความดันเลือดที่ สูงขึ้นอาจเนื่องจากวัดความดันเลือดที่เวลามากกว่า 4 ชั่วโมงหลังได้รับสมุนไพร นอกจากนี้ยังพบว่า high frequency power มีค่าเพิ่มขึ้นหลังการฝังเข็มร่วมกับได้รับสมุนไพรเมื่อเทียบกับก่อนและเทียบกับกลุ่ม sham ที่ ได้รับยา แสดงให้เห็นว่ายาสมุนไพรร่วมกับการฝังเข็มมีผลเพิ่มประสิทธิภาพการทำงานของระบบประสาทออโตโนมิก โดยการเพิ่มการทำงานของพาราซิมพาเทติก สรุปคือการฝังเข็มร่วมกับสมุนไพรจีนมีผลต่อระบบประสาทออโตโนมิก โดยเพิ่มการทำงานของพาราซิมพาเทติกโดยไม่มีผลต่อสมดุลของระบบประสาทออโตโนมิกและการทำงานของระบบประสาทซิมพาเทติก

Bai-Hu-Xian-Dan (Bai-Hu) is a new Chinese herbal formulation developed by the Faculty of Chinese Medicine at Huchiew Chalerm Prakiet University. It consists of twelve Chinese herbs, with three main components that improve the nervous, cardiovascular, and immune systems. However, it is not yet known whether this formulation can improve autonomic function. Therefore, the present study aims to investigate the effects of Bai-Hu-Xian-Dan (Bai-Hu), both alone and in combination with acupuncture (AC), on cardiac autonomic activity as assessed by heart rate variability (HRV). Subjects were divided into four groups: sham, sham with Bai-Hu, AC, and AC with Bai-Hu. The Bai-Hu treatment group received Bai-Hu in the morning, and 4 hours later, they were treated according to the experimental conditions, which included AC and Electrocardiogram (ECG) recording. ECG was recorded before, during, and after AC or herbal treatment. The results showed that diastolic blood pressure significantly decreased after AC compared to baseline. In the AC combined with Bai-Hu group, diastolic blood pressure significantly increased compared to the sham group. The decrease in diastolic blood pressure may be due to reduced peripheral resistance resulting from vasodilation, while the increase in this blood pressure might be attributed to the timing of the measurement, which was conducted 4 hours after herbal administration. This study also found that high frequency power significantly increased during and after AC combined with BaiHu. This suggests that AC and Bai-Hu can improve cardiac autonomic function by increasing parasympathetic activity. In conclusion, the combination of acupuncture and Bai-Hu can affect cardiac autonomic function by increasing parasympathetic activity without altering cardiac autonomic balance or sympathetic activity.
