รูปแบบการฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยการมีส่วนร่วมของชุมชน ตำบลบางโฉลง อำเภอบางพลี จังหวัดสมุทรปราการ

Abstract

การปฏิบัติเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบการฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยการมีส่วนร่วมของชุมชน ตำบลบางโฉลง อำเภอบางพลี จังหวัดสมุทรปราการ ดำเนินการระหว่างเดือนมกราคม-ตุลาคม 2557 โดยแบ่งเป็น 3 ระยะได้แก่ 1) ระยะการศึกษาสถานการณ์การฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้าน และ 2) ระยะการพัฒนารูปแบบการฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยการมีส่วนร่วมของชุมชน และ 3) ระยะการนำรูปแบบการฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยการมีส่วนร่วมของชุมชน ไปใช้และประเมินผล ผู้ให้ข้อมูลจำนวน 53 คน ประกอบด้วยผู้ให้ข้อมูลหลัก ได้แก่ ผู้ป่วยโรคหลอดเลือดสมงอ จำนวน 20 คน ผู้ให้ข้อมูลรอง ได้แก่ ผู้ดูแลหลักจำนวน 20 คน แกนนำชุมชนจำนวน 8 คน และบุคลากรสุขภาพจำนวน 5 คน เก็บข้อมูลโดยการสัมภาษณ์เจาะลึก การสังเกตแบบมีส่วนร่วมและไม่มีส่วนร่วม และการสนทนากลุ่ม วิเคราะห์ข้อมูลด้วยการวิเคราะห์เชิงเนื้อหา ผลการวิจัยพบว่า รูปแบบการฟื้นฟูสมรรถภาพผู้ป่วยโรคหลอดเลือดสมองที่บ้าน โดยการมีส่วนร่วมของชุมชน ตำบลบางโฉลง อำเภอบางพลี จังหวัดสมุทรปราการ ประกอบด้วย 2 แนวทาง คือ 1) แนวทางการวางแผนจำหน่ายผู้ป่วยเพื่อการฟื้นฟูสมรรถาภพต่อเนื่องที่บ้าน และ 2) แนวทางการฟื้นฟูสมรรถภาพผู้ป้วยโรคหลอดเลือดสมองที่บ้านโดยการมีส่วนร่วมของชุมชน ประเมินผลโดยรวม พบว่า ผู้ป่วยได้รับการฟื้นฟูสมรรถภาพที่บ้ายอย่างต่อเนื่อง ไม่เกิดภาวะแทรกซ้อน ผู้ดูแลหลักมีความพึงพอใจต่อบริการที่ได้รับ มีการช่วยเหลือและฟื้นฟูสมรรถภาพให้ผู้ป่วย แกนนำชุมชนเกิดความพึงพอใจที่ได้มีส่วนร่วมดำเนินงานและให้การสนับสนุนอุปกรณ์เพื่อช่วยในการออกกำลังกายและบริหารข้อต่อต่างๆ ที่เหมาะสม บุคลากรสุขภาพมีแนวทางในการเยี่ยมบ้านที่ทำให้ผู้ป่วยโรคหลอดเลือดสมองมีการฟื้นฟูสมรรถภาพที่ต่อเนื่อง

The purpose of this action research aimed to develop a model of rehabilitation for carebrovascular accidents (CVA) patients at home by community participation, Bangchalong Sub-District, Bangphi District, Samutprakan Province. The research had been conducted during January to October 2014. There were three phases of this research. First phase was study current situation of home rehabilitation services for CVA patients. Second phase was developing the rehabilitation model for CVA patients at home by community participation. The last phase was applying and evaluating a model of rehabilitation for CVA patients at home. A total of 53 informants included 20 CVA patients survivors others included 20 caregivers, 8 community leaders and 5 health care providers. Acquisition of data participation observation, in-depth interviews, and focus group discussion. Content analysis was employed as major method for data analysis. The study has found that a model of rehabilitation for CVA patients at home by community participation is composed of 2 directives 1) Guidelines of discharge plan for patients to continue the rehabilitation of patients at home and 2) Guidelines of the rehabilitation for CVA patients at home by community participation. Overall evaluation indicated find the patients received a continuing rehabilitation and did not experience additional complications. The caregivers were satisfied with the services received and were able to provide continuing patient care and rehabilitation activities. The community leaders were satisfied to participation the rehabilitation program and had provided the necessary equipments to assist them in doing physical and joint exercises. The health care providers had a home visit guideline that enabled them to provide continuing home rehabilitation to the patients.
