การพัฒนาแนวปฏิบัติทางการพยาบาลเพื่อฟื้นฟูสมรรถภาพปอดและส่งเสริมคุณภาพชีวิตของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง

Abstract

การศึกษาครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติทางการพยาบาลเพื่อฟื้นฟูสมรรถภาพปอดและส่งเสริมคุณภาพชีวิตของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง โดยใช้รูปแบบการปฏิบัติตามหลักฐานเชิงประจักษ์ของศูนย์ปฏิบัติการพยาบาลขั้นสูง ประเทศสหรัสอเมริกา ซึ่งมีขั้นตอนเริ่มต้นด้วยการวิเคราะห์ปัญหาในการปฏิบัติงานของพยาบาลที่คลินิกโรคปอดอุดกั้นเรื้อรัง แผนกผู้ป่วยนอก โรงพยาบาลบางปะอิน ด้วยการค้นหาหลักฐานเชิงประจักษ์โดยกำหนดคำสำคัญ เพื่อสืบค้นจากระบบฐานข้อมูล Sciencedirect, PubMED, บรรณานุกรมท้ายเล่ม ได้งานวิจัยระหว่างปี พ.ศ. 2544 - 2552 ที่ตรงประเด็นปัญหา จำนวน 8 เรื่อง โดยมีการศึกษารายงานการสังเคราะห์วรรณกรรมอย่างเป็นระบบจำนวน 4 เรื่อง และการวิจัยเชิงทดลองจำนวน 3 เรื่อง การวิจัยเชิงคุณภาพจำนวน 1 เรื่อง นำมาประเมินความน่าเชื่อถือของหลักฐานข้อมูล จากนั้นทำการวิเคราะห์และสังเคราะห์ งานวิจัยดังกล่าว จนได้แนวปฏิบัติทางการพยาบาลการฟื้นฟูสมรรถภาพปอดเพื่อส่งเสริมคุณภาพชีวิตของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง เครื่องมือได้ผ่านการตรวจสอบความเที่ยงตรงโดยผู้ทรงคุณวุฒิ 4 ท่าน จึงนำไปทดลองใช้แล้วนำมาปรับปรุงอีกครั้งจนได้แนวปฏิบัติประกอบด้วย 1. การประเมิน ข้อมูลทั่วไปของผู้ป่วย 2. การใช้แบบประเมินความรุนแรงของโรค 3. การให้ความรู้เรื่อง แนวทางการดูแลตนเองของผู้ป่วยโรคปอดอุดกั้นเรื้อรังได้แก่ฝึกการใช้ยาพ่นอย่างถูกวิธี ฝึกการไออย่างถูกวิธี ฝึกการหายใจอย่างมีประสิทธิภาพ และการออกกำลังกายภาย หลังการให้ความรู้ มีการ ประเมินผล ดังนี้ 1. การติดตามผลการดูแลตนเองทางโทรศัพท์ 2. การเยี่ยมบ้าน 3. ประเมินคุณภาพ ชีวิตของผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่คลินิก ข้อเสนอแนะแนวปฏิบัติการพยาบาลการฟื้นฟูสมรรถภาพปอดเพื่อส่งเสริมคุณภาพชีวิตของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง นี้สามารถนำไปใช้เป็นส่วนหนึ่งของการพัฒนาคุณภาพการบริการของพยาบาลเวชปฏิบัติได้จริง

The purpose of this study is to develop clinical nursing practice guideline (CNPG) for lung rehabilitation and promoting quality of life in chronic obstructive pulmonary disease (COPD) patients by adapting evidence based practice model of the Advanced Nursing Practice Center of U.S.A. The development began with analysis of the problems of nursing practice at clinic COPD, out patient department, Bang Pa In Hospital. Keywords were specified in the search of relevant evidences from the research study of Sciencedirect, PubMED, Reference list between the years 2001 - 2009 eight related literatures, which consisted of four Systematic review, three Experimental study and one Qualitative Research were selected to determine on reliability of evidences. From the analysis and synthesis of the said research literatures, the CNPG for lung rehabilitation and promoting quality of life in COPD patients. The CNPG was validated by three experts before performing to the patients. The CNPG consists of 1) Assess the patients' personal data. 2) Severity and progression of disease. 3) Health education guidelines for self care management which consist of using Metered dose inhaler (MDI), effective cough, breathing exercise, physical exercise. The assessment were 1) Follow up by phone call and motivating for self care. 2) Home visits for the patients assessment about self care. 3) Evaluate the quality of life of the COPD patients at the clinic. Recommendation to evaluate the CNPG, it should perform for longer and larger amount of COPD patients and develop to lung rehabilitation and promoting quality of life. Further more, to integrate the CNPG as part of a quality care development of the health center.
