การวิจัย เรื่อง "การเข้าไม่ถึงบริการทางการแพทย์ : ศึกษาเฉพาะกรณีคนพิการในสถานสงเคราะห์คนพิการและทุพพลภาพพระประแดง จังหวัดสมุทรปราการ" มีวัตถุประสงค์เพื่อศึกษาสิทธิทางการแพทย์ของคนพิการในสถานสงเคราะห์ ศึกษาบริการทางการแพทย์สำหรับคนพิการที่สถานสงเคราะห์จัดให้ ศึกษาปัญหาและสาเหตุของการเข้าไม่ถึงบริการทางการแพทย์ของคนพิการในสถานสงเคราะห์ และเพื่อแสวงหาแนวทางและมาตรการให้คนพิการเข้าถึงบริการทางการแพทย์ การวิจัยครั้งนี้เป็นการวิจัยเชิงคุณภาพ โดยทำการสัมภาษณ์เชิงลึกกลุ่มคนพิการในสถานสงเคราะห์จำนวน 5 คน เจ้าหน้าที่พี่เลี้ยงจำนวน 4 คน บุคลากรที่เกี่ยวข้องกับการบริการทางการแพทย์ในสถานสงเคราะห์จำนวน 4 คน และหัวหน้าฝ่ายสวัสดิการสงเคราะห์จำนวน 1 คน รวมทั้งสิ้น 14 คน ผลการศึกษาพบว่า การรับรู้สิทธิทางการแพทย์ของคนพิการและเจ้าหน้าที่พี่เลี้ยงที่ดูแลใกล้ชิดคนพิการมีค่อนข้างจำกัด โดยจะทราบเกี่ยวกับการที่คนพิการสามารถไปรักษาได้ฟรีทุกโรงพยาบาลและการได้รับเบี้ยยังชีพสำหรับคนพิการเดือนละ 500 บาทเป็นหลัก ในทางกลับกันคนพิการและเจ้าหน้าที่ของสถานสงเคราะห์ทุกคนรู้สึกว่าคนพิการถูกเลือกปฏิบัติและถูกจำกัดสิทธิในเรื่องของการเข้ารับบริการทางการแพทย์เป็นอย่างมาก ทั้งยังถูกปฏิบัติโดยไม่คำนึงถึงศักดิ์ศรีความเป็นมนุษย์ ถูกแบ่งแยกและไม่ได้รับความสะดวกในการเข้าถึงบริการทางการแพทย์ ส่วนบริการทางการแพทย์ที่สถานสงเคราะห์จัดให้นั้น พบว่า แม้เป็นบริการที่จัดคนพิการในสถานสงเคราะห์ตามความจำเป็นแต่เป็นไปตามหลักสิทธิคนพิการ ผลการศึกษายังพบอีกว่าปัญหาและสาเหตุของการเข้าไม่ถึงบริการทางการแพทย์ของคนพิการมาจากจำนวนคนพิการที่มีความต้องการรับบริการทางการแพทย์เพิ่มสูงขึ้นแต่บุคลากรที่ให้บริการทางการแพทย์มีอยู่อย่างจำกัดส่งผลให้เกิดความล่าช้า การนำส่งไปรับบริการทางการแพทย์ภายนอกจะถูกพิจารณาตามลำดับความสำคัญจากหน้าที่พยาบาล ไม่สามารถจัดบริการตามความต้องการที่แท้จริงของคนพิการ สาเหตุที่สำคัญอีกประการหนึ่งที่พบว่าเป็นอุปสรรคในการนำส่งคนพิการไปรับบริการทางการแพทย์ภายนอก คือ กฎเกณฑ์การให้บริการที่ต้องมีบัตรประกันสุขภาพถ้วนหน้าตามระเบียบที่กำหนดโดยกระทรวงสาธารณสุข คนพิการจะสามารถเข้ารับบริการได้ต้องมีหลักฐานแสดงตน แต่กลุ่มคนพิการของสถานสงเคราะห์ส่วนใหญ่ประสบปัญหาไม่มีหลักฐานแสดงตน ถือเป็นปัจจัยสำคัญที่ทำให้คนพิการไม่สามารถเข้าถึงบริการทางการแพทย์ได้ในทางปฏิบัติ ในกลุ่มคนพิการที่มีสิทธิบัตรประกันสุขภาพตามที่กำหนดแล้วก็พบว่ามีขั้นตอนการให้บริการที่ซับซ้อนยากที่คนพิการจะเข้าถึง และยังคงมีการเลือกปฏิบัติต่อคนพิการ การศึกษานี้เสนอแนวทางและมาตรการในการแก้ไขปัญหาการเข้าไม่ถึงบริการทางการแพทย์ว่า ควรมีมาตรการที่เหมาะสมในการอำนวยความสะดวกแก่คนพิการ ร่วมกับการกำหนดผู้ประสานงานหรือรับผิดชอบดูแลให้ความช่วยเหลือคนพิการในหน่วยบริการทางการแพทย์อย่างชัดเจน และการให้บริการที่คำนึงสิทธิของคนพิการเป็นหลัก ข้อค้นพบจากการศึกษาครั้งนี้ ผู้วิจัยมีข้อเสนอแนะในระดับนโยบายให้กระทรวงพัฒนาสังคมและความมั่นคงของมนุษย์ กระทรวงสาธารณสุข และกระทรวงมหาดไทยซึ่งมีความเกี่ยวข้องโดยตรงกับการเข้าถึงบริการทางการแทพย์ของคนพิการ แต่ต่างก็มีพระราชบัญญัติที่บังคับใช้เป็นของตนเอง จนทำให้เกิดความซับซ้อนกลายเป็นอุปสรรคเมื่อนำไปปฏิบัติจริง จึงควรมีการจัดระบบการทำงานร่วมกันโดยใช้หลักบูรณาการที่มีเป้าหมายเพื่อประโยชน์ต่อคนพิการเป็นสำคัญ รวมทั้งควรทบทวนและทำความเข้าใจกับหน่วยงานที่เกี่ยวข้องทั้งภาครัฐและเอกชนโดยเฉพาะอย่างยิ่งในหน่วยที่ให้บริการทางการแพทย์อย่างเร่งด่วน ให้ตระหนักถึงความสำคัญในการให้บริการแก่คนพิการโดยคำนึงถึงสิทธิของคนพิการเป็นสำคัญเพื่อขจัดปัญหาและอุปสรรคของการเข้าไม่ถึงบริการทางการแพทย์ตามที่นโยบายได้กำหนดไว้
This research on "The Inaccessibility to Medical Services : A Case Study of People with Disability in Phrapradaeng Home for Diabled Persons", is conducted to study medical rights of person with disavilities in Phrapradaeng Home, to investigate medical services for people with disabilities, to study problems and causes of inaccessibility to medical services, and to seek ways and measures enabling prople with disabilities to be accessible to medical services. The research methodology employed qualitative methods by in-depth interviews with 14 staff in Phrapradaeng Home for disabled persons, namely, 5 disabled people, 4 mentor officers, 4 officers working for medical services and 1 head of welfare assistance department. The finding indicates that persons with disabilities and mentor officer hardly realize medical rights for disabled people. They only are aware of free medical services in all hospitals and monthly allowance for the disabled at 500 Baht. On the contary, people with disabilities and officers feel that not only they are not well treated and their rights are very limited but also they are discriminated against bumanity and never find hospitals convenient for them. It is found that medical services in the home, is provided to serve the disabled's demand only and to be in accordance with the disabled person's rights. The stidy also finds that the problem of inaccessibility to medical services of people with disabilities comes from an increase of disbled patients while a number of medical personnel is still limited and thus leads to slow services. In general hospitals, nursing personnel is responsible for patient servicing and does not prioritize the needs of persons with disabilities. Another important reason whicj does not facilitate the disabled in general hospital is health insurance card issued by Ministry of Public Health. People with disabilities need to present health cards which, unfortunately, most of them have not had yet and then are not entitled for medical services. Health insurance card procedure is too complicated for the disabled and discriminated. This study proposed measures to resolve the problem of inaccessibility to medical services that there should be appropriate measures which facilitate persons with disabilities. Coordiantors or personnel in hospitals should be assigned to be directly responsible for people with disabilities and hospitals should provide medical services in accordance with the rights of the disabled. Based on the findings from this study, the researcher suggests corporate polict to Ministry of Social Development and Human Security, Ministry of Public Health and Ministru of Interior, all which are directly related to medical services of people with disabilities but each has its own enforcement causing complexity and hindrance when practiced. All the relevant ministries should seek cooperation and set the same goal for the disabled through integration management system. They also should urgently review and create understanding among relevant agencies in both public and private sectors, especially those that provide disabilities and their rights so as to eliminate problems and difficulties for such inaccessibility. For practical suggestions, medical unites should reduce steps in their services and provide more flexibility toward disabled people allowing them to be more accessible to medical services. The home itself should be provided with adequate officers to cater for the demand of the disabled. It should promote its personnel to study and understand the new policy. This is another direction to help reduce the problem of inaccessibility.
