งานวิจัยครั้งนี้เป็นการศึกษาต้นทุนในการรับบริการรักษาพยาบาลของผู้สูงอายุ จำแนกเป็นต้นทุนระยะสั้นและระยะยาว และเก็บข้อมูลทั้งต้นทุนทางตรงและทางอ้อมเพื่อให้ได้ความครอบคลุมของต้นทุนที่ผู้สูงอายุต้องจ่าย รวมถึงวิเคราะห์ความเต็มใจจ่ายเพื่อนำข้อมูลมาเสนอแนะรูปแบบการร่วมจ่ายในการรับบริการรักษาพยาบาลของผู้สูงอายุ เก็บข้อมูลผู้สูงอายุในจังหวัดสมุทรปราการ จำนวน 460 คน โดยใช้แบบสัมภาษณ์จากการศึกษาพบว่าบริการรักษาพยาบาลยังคงเป็นสินค้าจำเป็นสำหรับผู้สูงอายุ ค่าความยืดหยุ่นต่อรายได้ของต้นทุนในการดูแลสุขภาพแบบผู้ป่วยนอกมีค่าเท่ากับ 0.11 ผู้ป่วยในเท่ากับ 0.60 การไปรับบริการสุขภาพในแต่ละครั้งจะมีต้นทุนที่เกิดขึ้นทั้งต้นทุนทางตรงและต้นทุนทางอ้อม กรณีต้นทุนระยะสั้น ได้แก่การไปรับบริการแบบผู้ป่วยนอกจะมีต้นทุน 696 บาทต่อครั้ง ผู้ป่วยใน 3,692 บาทต่อครั้ง และไปรับบริการรักษาโรคเรื้อรัง 497 บาทต่อครั้ง ต้นทุนระยะยาวกรณีผู้สูงอายุที่จ้างผู้ช่วยเหลือมาดูแลที่บ้านจะมีต้นทุน 3118 บาทต่อเดือน ผู้สูงอายุที่ไปรับบริการที่ศูนย์ดูแลผู้สูงอายุจะมีต้นทุน 3,750 บาทต่อเดือน และผู้สูงอายุทั้ง 2 กลุ่มที่ช่วยเหลือตนเองไม่ได้จะมีต้นทุนในการซื้ออุปกรณ์ในการดูแลสุขภาพ 2,312 บาทต่อเดือน และลักษณะการเพิ่มขึ้นของต้นทุน พบว่าต้นทุนในการรักษาโรคเรื้อรังมีแนวโน้มเพิ่มขึ้นตามอายุ ต้นทุนผู้ป่วยนอกและผู้ป่วยในไม่เพิ่มขึ้นตามอายุ ส่วนต้นทุนในการดูแลสุขภาพระยะยาวจะมีลักษณะของการเพิ่มขึ้นตามอายุอย่างชัดเจน และสูงขึ้นอย่างมากเมื่ออายุ 90 ปีขึ้นไปจากการศึกษาความเต็มใจจ่ายพบว่าผู้สูงอายุร้อยละ 22 มีความเต็มใจจ่าย และเมื่อวิเคราะห์ปัจจัยที่มีผลต่อความเต็มใจจ่ายของผู้สูงอายุพบว่าอายุที่เพิ่มขึ้นและน้ำหนักที่เพิ่มขึ้นของผู้สูงอายุมีผลทำให้ผู้สูงอายุความเต็มใจจ่ายสูงขึ้น และนำผลการวิเคราะห์มาสร้างเป็นรูปแบบร่วมจ่าย ได้ผลว่าผู้สูงอายุกรณีน้ำหนักอยู่ในเกณฑ์ปกติควรจะมีการร่วมจ่ายเมื่อไปรับบริการครั้งละ 110 -160 บาท ซึ่งเป็นในลักษณะ progressive rate ตามอายุ แต่จากผลการวิจัยพบว่ารายได้ของผู้สูงอายุมีความแตกต่างกันค่อนข้างมาก ในการร่วมจ่ายจึงควรยกเว้นการร่วมจ่ายสำหรับผู้สูงอายุที่มีรายได้น้อย มุ่งเน้นการร่วมจ่ายในกลุ่มผู้สูงอายุที่มีความสามารถในการจ่ายเพื่อให้สถานบริการสุขภาพมีเงินเพียงพอเพื่อมาช่วยปรับปรุงระบบบริการให้ดีขึ้น หรืออาจมีการจัดตั้งเป็นกองทุนดูแลสุขภาพผู้สูงอายุ และมีการเก็บเงินสมทบกองทุนจากผู้สูงอายุเพื่อใช้ในการดูแลสุขภาพตนเอง เพื่อให้กองทุนมีรายได้และสามารถดูแลสุขภาพผู้สูงอายุได้อย่างยั่งยืน
This research aims to study the cost of receiving medical care for senior citizens, classified into acute care costs and long-term care costs. Both direct costs and indirect costs are analyzed in order to cover all the costs which the elderly have to pay. Willingness to pay on medical care is also included to find suggestions on methods for elderly medical care co-payment. The data is collected from 460 senior citizens in Samut Prakan province through interviews.The study shows that medical care is still a necessary good for elders. Flexibility per income of the medical cost for outpatients is 0.11 and inpatients is 0.60. There are both direct and indirect costs each time medical care is received. The cost for outpatients is 696 baht, inpatients is 3,693 baht and chronic disease patients is 497 baht, respectively, for acute care costs each treatment session. The long-term care cost to hire domestic assistance is 3118 baht per month. For senior citizens that receive care from a nursing home, the cost is 3,750 baht per month. Bedridden senior citizens from the two groups have a monthly health care equipment cost of 2,312 baht. For cost increase patterns, the cost for chronic disease patients tends to increase in correlation to the increase of age, whereas the cost for inpatient and outpatient care do not increase as the patient gets older. However, long-term care costs significantly increases in correlation with the increasing age of the patient and increases at a higher rate when the patient is over 90 year-old The study of the willingness to pay shows that 22 percent of elder citizens is willing to spend money on medical care. When the factors which impact their willingness to pay are analyzed, it was found that increased age and weight result in increased willingness to pay of the elderly citizens. The analysis results were created into a co-payment model; the study reveals that elders with average weight should co-pay for medical care at 110-160 baht per treatment session in a progressive rate according to age.
