ทฤษฎีการเปลี่ยนผ่าน : กรณีศึกษาสตรีตั้งครรภ์ปกติ

Abstract

ทฤษฎีการเปลี่ยนผ่านเป็นทฤษฎีระดับกลางที่ผู้วิจัยและนักปฏิบัติการพยาบาลทางคลินิกสามารถเข้าถึงและนำไปประยุกต์ใช้แก่ผู้รับบริการ สตรีตั้งครรภ์เป็นผู้รับบริการกลุ่มหนึ่งที่จะต้องได้รับการดูแลสุขภาพเป็นอย่างดี ทั้งนี้เนื่องจากการเปลี่ยนแปลงได้เกิดขึ้นอย่างมากมายในขณะตั้งครรภ์ทั้งทางร่างกายและจิตใจ ยิ่งไปกว่านั้น การตั้งครรภ์ก็เป็นการเปลี่ยนผ่านที่เกิดขึ้นตามพัฒนาการหรือในช่วงชีวิตหนึ่งของสตรี ซึ่งส่งผลทั้งทางบวกและทางลบต่อสตรีตั้งครรภ์ ทารกในครรภ์ และครอบครัวได้ จากกรณีศึกษาสตรีตั้งครรภ์ปกติ พบว่า การรับรู้เกี่ยวกับการตั้งครรภ์ ตลอดจนการรับรู้ถึงการเปลี่ยนแปลงที่เกิดขึ้นกับตนเองขณะตั้งครรภ์ ส่งผลให้สตรีตั้งครรภ์ให้ความสนในในการดูแลตนเองขณะที่ตั้งครรภ์ นอกจากนี้ การได้รับการดูแลเอาใจใส่เป็นอย่างดีจากสามีและครอบครัวทั้งทางด้านร่างกาย จิตใจ และสังคมเศรษฐกิจ อีกทั้งความห่วงใยที่ได้รับจากญาติและเพื่อนบ้านที่อยู่ใกล้เคียงเป็นเงื่อนไขหนึ่งที่เอื้ออำนวยให้สตรีตั้งครรภ์ปกติมีกำลังกายและกำลังใจที่จะช่วยให้การเปลี่ยนผ่านดำเนินการไปด้วยดี สำหรับการบำบัดทางการพยาบาล พยาบาลเป็นบุคคลสำคัญที่มีบทบาทหน้าที่ในการประเมินความพร้อม การเตรียมการ ตลอดจนการใช้บทบาทเสริม ซึ่งบทบาทในการดูแลสตรีตั้งครรภ์ จะอยู่ในรูปแบบของการประเมินภาวะสุขภาพของสตรีตั้งครรภ์และทารกในครรภ์ การตรวจร่างกาย การตรวจครรภ์ การให้คำแนะนำในการปฏิบัตตัว รวมถึงการให้คำแนะนำในการปฏิบัติตัวเพื่อบรรเทาอาการไม่สุขสบายที่อาจเกิดขึ้นในขณะตั้งครรภ์ ทั้งนี้เพื่อให้การเปลี่ยนผ่านของสตรีตั้งครรภ์ดำเนินไปได้อย่างราบรื่น

Transition theory is a middle-range theory, which makes researchers and clinical nurses able to access and apply to serve their clients. Pregnant women is a group of clients, which need to receive a good health care because many changes occur during pregnancy, both physiological and psychological. Moreover, pregnancy is also a transition, which follows a developmental process or one moment of a woman’s life. All of these have both positive and negative effects on pregnant women, fetus, and family. According to a case study of a normal pregnant woman, it was found that awareness of pregnant women pay attention to taking care of themselves. In addition, receiving good care from husband and family both physical, psychological, socioeconomic, along with receiving caring from cousins and neighbors were the facilitating conditions. These facilitating conditions could give pregnant women both physiological and psychological support in order to help them to get through their transition healthily. For nursing therapeutics, nurses are the important persons, who have roles and duties in terms of assessment of readiness, preparation, and role supplementation. These roles include health assessment of both pregnant women and fetus, physical examination, abdominal examination and advising pregnant women how to behave or to relieve unpleasant symptoms. All of these roles would help pregnant women to go through this transition smoothy.
