ปัจจัยที่มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่ายของผู้ประกันตน

Abstract

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่ายของผู้ประกันตน โดยศึกษาความสัมพันธ์ของปัจจัยต่าง ๆ ได้แก่ ปัจจัยส่วนบุคคล ปัจจัยด้านการรับรู้ลักษณะการเจ็บป่วยและการประเมินการเจ็บป่วยในการตัดสินใจใช้บริการทางการแพทย์ ปัจจัยด้านการรับรู้สิทธิประโยชน์การใช้บริการทางการแพทย์ และปัจจัยด้านทัศนคติที่มีต่อสถานพยาบาลเครือข่าย นอกจากนี้ยังศึกษาคุณภาพบริการทางการแพทย์ของสถานพยาบาลเครือข่าย กลุ่มตัวอย่างที่ใช้ในการศึกษา คือ ผู้ประกันตนที่ขึ้นทะเบียนกับโรงพยาบาลสมเด็จพระบรมราชเทวีณ ศรีราชา เป็นสถานพยาบาลคู่สัญญาหลักและมารับบริการทางการแพทย์กรณีผู้ป่วยนอกในโรงพยาบาลสมเด็จพระบรมราชเทวีณ ศรีราชา ด้วยโรคที่สามารถรักษาได้ที่สถานพยาบาลเครือข่าย จำนวน 384 ราย เก็บข้อมูลระหว่างเดือนมีนาคมถึงสิงหาคม พ.ศ. 2550 เครื่องมือที่ใช้ในการวิจัยเป็นแบบสอบถาม 4 ชุด ที่มีค่าความเชื่อมั่น 0.78, 0.80, 0.76 และ 0.82 เก็บข้อมูลโดยการสัมภาษณ์ ผลการศึกษา พบว่า กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง อายุระหว่าง 30-44 ปี สถานภาพสมรส (คู่) พักอาศัยอยู่ในอำเภอศรีราชา จังหวัดชลบุรีการศึกษาระดับมัธยมศึกษา รายได้เฉลี่ยต่อเดือน 5,001-10,000 บาท รับบริการที่โรงพยาบาลด้วยอาการแสดงและสิ่งผิดปกติที่พบจากการตรวจทางคลินิก ไม่ได้ไปรับบริการจากที่อื่นก่อนมารับบริการในครั้งนี้ และทราบว่า ถ้าเลือกโรงพยาบาลสมเด็จพระบรมราชเทวีณ ศรีราชา เป็นสถานพยาบาลหลักแล้ว สามารถไปใช้บริการสถานพยาบาลเครือข่ายได้ผู้รับบริการทราบข้อมูลสถานพยาบาลเครือข่ายจากบริษัท/หน่วยงานที่ทำงานอยู่ ช่วงเวลาที่กลุ่มตัวอย่างสะดวกไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่ายมากที่สุด คือ ช่วงเวลา 17.00 – 22.00 น. การวิเคราะห์ความสัมพันธ์ปัจจัยส่วนบุคคล ได้แก่ เพศ อายุ สถานภาพสมรส ที่พักอาศัยระดับการศึกษา รายได้ พบว่า ไม่มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่าย ส่วนปัจจัยด้านการรับรู้ลักษณะการเจ็บป่วยและการประเมินการเจ็บป่วย ปัจจัยด้านการรับรู้สิทธิประโยชน์การใช้บริการทางการแพทย์และปัจจัยด้านทัศนคติที่มีต่อสถานพยาบาลเครือข่าย มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่าย อย่างมีนัยสำคัญทางสถิติที่ระดับ α = 0.05 คุณภาพการบริการทางการแพทย์ของสถานพยาบาลเครือข่ายโดยรวมทุกด้าน อยู่ในระดับปานกลาง

The objectives of this study were to study factors related to the non-using medical services at network hospitals of insured persons registered at Queen Savang Vadhana Memorial Hospital with the relationship of relevant factors. The studied factors included the personal factor, the perception of illness and the illness evaluation prior to the medical service acceptation, the perceiving of the right factor of assertion under the network hospitals, the factor of attitude towards the network hospitals and the medical service quality. A total of 384 insured persons with mild and moderate illnesses who have registered at Queen Savang Vadhana Memorial Hospital were studied from March to August 2007. Four questionnaires were used by the interviewers as the research tools with reliabilities of 0.78, 0.80, 0.76 and 0.82 respectively. Results showed that the majority of participants were female with married status. Their ages range from 30 to 44 years. Their residences were mostly located in Sriracha District, Chonburi province. Most of them finished high school level, and the average income is 5,001-10,000 bath per month. The illnesses, symptoms and abnormalities of the samples were confirmed by clinical checking. No other medical checking was made before receiving this medical service. The samples have know that they could receive the medical services from any other network hospitals under the condition of selecting the Queen Savang Vadhana Memorial Hospital as the main contractor. They learned the information of network hospitals from their current working companies. The most convenient time for receiving the medical services was 5.00-10.00 p.m. The analysis of relationship between sex, age, marital status, residence, education background and income found no association with non-using medical services at network hospitals. However, the illness perception, the illness evaluation, the perceiving of the right factors of assertion under the network hospitals and the factors of attitude towards network hospitals were found significantly relating to non-using medical services at α = 0.05. The overall medical service quality of the network hospitals was found fair.
