รูปแบบการบริหารจัดการรายกรณีเพื่อการคุ้มครองเด็กที่ถูกกระทำความรุนแรงในสังคมไทย

Abstract

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาพัฒนาการและแนวคิดการบริหารจัดการรายกรณี รูปแบบการบริหารจัดการรายกรณี และพัฒนารูปแบบการบริหารจัดการรายกรณี เพื่อคุ้มครองเด็กที่ถูกกระทำความรุนแรงเชิงบูรณาการในสังคมไทย เป็นการวิจัยเชิงคุณภาพโดยการศึกษาข้อมูลจากเอกสาร การศึกษาภาคสนาม โดยใช้เทคนิคในการเก็บรวบรวมข้อมูล ได้แก่ การสัมภาษณ์เจาะลึกผู้บริหารและผู้ปฏิบัติงานคุ้มครองเด็กที่ถูกกระทำความรุนแรงของหน่วยงานภาครัฐ ภาคเอกชนและท้องถิ่น และการจัดสนทนากลุ่มผู้มีส่วนเกี่ยวข้องใน 3 พื้นที่ คือ เทศบาลตำบลท่างิ้ว อำเภอห้วยยอด จังหวัดตรัง องค์การบริหารส่วนตำบลดอนแก้ว อำเภอแม่ริม จังหวัดเชียงใหม่ และองค์การบริหารส่วนตำบลกุดยาลวน อำเภอตระการพืชผล จังหวัดอุบลราชธานี และทีมสหวิชาชีพ ตลอดจนการศึกษารูปแบบการจัดการรายกรณีของหน่วยงานด้านเด็ก ประเทศสหรัฐอเมริกา ด้วยวิธีการสังเกตแบบไม่มีส่วนร่วม เพื่อนำข้อมูลมาวิเคราะห์ร่วมต่อการพัฒนารูปแบบการบริหารจัดการรายกรณีเพื่อคุ้มครองเด็กที่ถูกกระทำความรุนแรงในสังคมไทยต่อไป ผลการศึกษาพบว่า พัฒนาการจัดการรายกรณีเพื่อการคุ้มครองเด็กในประเทศไทย เริ่มในปี 2550 หลังเกิดเหตุการณ์สึนามิ โดยองค์การทุนเพื่อเด็กแห่งสหประชาชา (ยูนิเซฟประเทศไทย) ร่วมกับภาครัฐจัดอบรมผู้จัดการรายกรณีในชุมชนแก่เจ้าหน้าที่ขององค์กรปกครองส่วนท้องถิ่นใน 6 จังหวัดภาคใต้ เพื่อทำหน้าที่คุ้มครองและช่วยเหลือเด็กที่ได้รับผลกระทบจากเหตุการณ์สึนามิ ต่อมาได้ขยายการอบรมไปจังหวัดอื่นทั่วประเทศ ซึ่งในแต่ละจังหวัดที่ได้รับการอบรมสามารถนำองค์ความรู้ไปปรับใช้ภายใต้บริบทของพื้นที่ซึ่งมีกลไกส่งเสริมการดำเนินการจัดการบริหารจัดรายกรณี เพื่อการคุ้มครองเด็กที่แตกต่างกัน ได้แก่ ด้านกฎหมายและอำนาจทางกฎหมาย นโยบาย ด้านเครือข่ายชุมชน และด้านทรัพยากรบุคคล และจากผลการศึกษาการบริหารจัดการรายกรณีของประเทศไทย ผนวกรวมกับการศึกษาการบริหารจัดการรายกรณี ณ San Diego Youth Services ประเทศสหรัฐอเมริกา ทำให้ได้ข้อเสนอแนะต่อการพัฒนารูปแบบการบริหารจัดการรายกรณีในประเทศไทย ประกอบด้วย 1) รูปแบบการดำเนินการในชุมชนระดับตำบล 2) รูปแบบการดำเนินการภายในองค์กร และ 3) รูปแบบการดำเนินการระดับจังหวัดของทีมสหวิชาชีพ และข้อเสนอแนะต่อกระทรวงการพัฒนาสังคมและความมั่นคงของมนุษย์ คือ ควรกำหนดและพัฒนายุทธศาสตร์การดูแลและคุ้มครองเด็กในระยะยาว มีนโยบายความร่วมมือให้บูรณาการงานของทุกกระทรวง ทบวง กรม สนับสนุนให้มีเจ้าภาพดำเนินงานด้านการดูแลและคุ้มครองสวัสดิภาพเด็กทุกระดับ ส่งเสริมให้ชุมชนและครอบครัวเข้ามามีส่วนร่วม มีการใช้เครื่องมือปฏิบัติงาน พัฒนาระบบงานการจัดเก็บข้อมูล จัดให้มีระบบการนิเทศงาน และควรพัฒนาบุคลากรอย่างต่อเนื่อง

The research objectives to study the development and concepts of case management and to develop the integrated model of case management for child abuse protection in Thai society. This research is a Qualitative Research. Documentary study and field research was used for answer the research questions; Field Research in Thailand includes in-depth interview administrator, staff in child abuse protection agency and focus group discussion with child abuse professionals from organizations such as the Tha Ngiew sub-district Administrative Organization, Huai Yoy district, Trang province, Don Kaew sub-district Administration Organization. Mae Rim district, Chiang Mai province, Kut Ya Luan sub-district Administration Organization, Trakan Phuet Phon district, and Ubon Ratchathani province and multidisciplinary team. Additionale, Trakan Phuet Phon district, and Ubon Ratchathani province and multidisciplinary team. Additionally, non-participant observation and examination of case management model in the United States with the San Diego Youth Services also help to provide a comparative framework for further developing child abuse case management model in Thailand. Findings of the research: Case management for protecting children in communities in Thailand had started after the incident of Tsunami in 2004 with the support from UNICEF Thailand with Ministry of Social Development and Human Secutiry, to arrange Case Manager Training for Local Administration Officers in order to provide the assistance and Child Protection in the community which impacted from Tsunami in the Southern Region and expanded to other provinces. Each area to apply in different contexts such as policies, laws, legal authority, network community and human resources. Based on findings of field research in Thailand and case manage model of San Diego Youth Services in USA, developed the integrated model of case management for child abuse protection in Thai society, which include case management in 3 levels : 1) case management operation at Sub-district level 2) case management operation in within an organization and 3) case managements of interdisciplinary team at the provincial level. Recommendations: Ministry of Social Development and Human Security should set up long-term and practical child protection strategies that cover policies and operators in all levels. Support the community and family involvement and tool for intervention. Support case management supervision structure and support skill for case manager.
