ความเสี่ยงต่อภาวะแทรกซ้อนสำคัญในผู้ป่วยโรคเบาหวานและความดันโลหิตสูง ที่ รพ.สต. หนองปรือ อำเภอบางพลี จังหวัดสมุทรปราการ

Abstract

โรคเบาหวานและโรคความดันโลหิตสูงจัดอยู่ในกลุ่มโรคไม่ติดต่อ(Non-communicable diseases: NCDs) มีความเสี่ยงสูงต่อการเกิดโรคไตเรื้อรัง โรคหลอดเลือดสมอง โรคหัวใจและหลอดเลือดที่เป็นปัญหา สำคัญทางด้านสาธารณสุข วัตถุประสงค์ของการวิจัย เพื่อศึกษาความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือด โรคไตวายเร้ือรังโรคหลอดเลือดสมองของผู้ป่วยโรคเบาหวานและความดันโลหิตสูง ที่รับการรักษาที่โรงพยาบาลส่งเสริมสุขภาพตำบลหนองปรือ อำเภอบางพลี จังหวัดสมุทรปราการ กลุ่มตัวอย่าง คือ ผู้ป่วยที่เป็นโรคเบาหวาน โรคความดันโลหิตสูง หรือผู้ป่วยที่เป็นทั้งโรคเบาหวานและโรคความดันโลหิตสูง ที่มารักษาอย่างต่อเนื่องที่โรงพยาบาลส่งเสริมสุขภาพตำบลหนองปรือ จำนวน 301 คน เครื่องมือที่ใช้ในการวิจัย คือ Stroke risk scorecard, Thai CV risk score และ Thai CKD risk score วิเคราะห์ข้อมูลโดยใช้สถิติ ความถี่ และร้อยละ

ผลการวิจัย พบว่า กลุ่มตัวอย่างทั้งหมด 301คน เป็นเพศชายร้อยละ 33.55 และเพศหญิงร้อยละ 66.45 โดยมีอายุมากกว่าหรือเท่ากับ 60 ปีร้อยละ 69.44 มีโรคประจำตัว คือ โรคความดันโลหิตสูง ร้อยละ 55.82 โรคเบาหวาน ร้อยละ7.30 และโรคเบาหวานและความดันโลหิตสูง ร้อยละ 36.88

กลุ่มตัวอย่างที่เป็นทั้งโรคความดันโลหิตสูงและโรคเบาหวานซึ่งใช้ผลตรวจทางห้องปฏิบัติการ และไม่ใช้ผลการตรวจทางห้องปฏิบัติการในการประเมินความเสี่ยง พบว่า ร้อยละ 32 มีความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในระดับปานกลาง และร้อยละ 34.65 มีความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในระดับเสี่ยงสูงมากตามลำดับ มีความเสี่ยงต่อการเกิดโรคไตเรื้อรังในระดับเสี่ยงสูงมาก ร้อยละ 58.25 และ 76.85 ตามลำดับ และมีความเสี่ยงอยู่ในระดับเสี่ยงปานกลาง และเสี่ยงสูงต่อการเกิดโรคหลอดเลือดสมองร้อยละ 50 เท่ากัน

Diabetes mellitus and hypertension are classified in a group of non-communicable diseases that are high risk for chronic kidney diseases, cerebrovascular diseases, and cardiovascular diseases. All are major problems in public health. This research aimed to examine the risks of cardiovascular diseases, chronic kidney diseases, and cerebrovascular diseases among patients with diabetes and hypertension in Nong-Prue Health Promoting Hospital, Bangphli, Samutprakan. A sample, 301 persons, was selected from patients with diabetes, hypertension, or both, who regularly got treatment at Nong-Prue Health Promoting Hospital. The measurements were Stroke risk scorecard, Thai CV risk score, and Thai CKD risk score. Data were analyzed using frequency and percentage.

The results showed that 301 samples comprised 33.55% male and 66.45% female, 69.44% over or equal 60 year-old of age. Regarding underlying diseases, there were hypertension (55.82%), diabetes mellitus (7.30%), and both diseases (36.88%).

The findings of patients with hypertension and diabetes with and without laboratory tests for screening found that 32% got moderate risk and 34.65% showed very high risk for cardiovascular diseases, respectively; 58.25% and 76.85% revealed very high risk for chronic kidney disease, respectively; and showed moderate risk and high risk of stroke by 50%.
