ปัจจัยที่มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแทพย์ที่สถานพยาบาลเครือข่ายของผู้ประกันตนโรงพยาบาลสมเด็จพระาบรมราชเทวี ณ ศรีราชา

Abstract

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการไม่ไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่ายของผู้ประกันตนโรงพยาบาลสมเด็จพระบรมราชเทวี ณ ศรีราชา โดยศึกษาความสัมพันธ์ของปัจจัยต่างๆ ได้แก่ ปัจจัยส่วนบุคคล ปัจจัยด้านการรับรู้ ลักษณะการเจ็บป่วย และการประเมินการเจ็บป่วยในการตัดสินใจใช้บริการทางการแพทย์ ปัจจัยด้านการรับรู้สิทธิประโยชน์การใช้บริการทางการแพทย์ และปัจจัยด้านทัศนคติที่มีต่อสถานพยาบาลเครือข่าย นอกจากนี้ยังศึกษาคุณภาพบริการทางการแพทย์ของสถานพยาบาลเครือข่าย กลุ่มตัวอย่างที่ใช้ในการศึกษา คือ ผู้ประกันตนที่ขึ้นทะเบียนกับโรงพยาบาลสมเด็จพระบรมราชเทวี ณ ศรีราชา เป็นสถานพยาบาลคู่สัญญาหลัก และมารับบริการทางการแพทย์กรณีผู้ป่วยนอกในโรงพยาบาลสมเด็จพระบรมราชเทวี ณ ศรีราชา ด้วยโรคที่สามารถรักษาได้ที่สถานพยาบาลเครือข่าย จำนวน 384 คน เก็บข้อมูลระหว่างเดือนมีนาคมถึงสิงหาคม 2550 เครื่องมือที่ใช้ในการวิจัยเป็นแบบสอบถาม 4 ชุด ที่มีค่าความเชื่อมั่น 0.78, 0.80, 0.76, 0.82 เก็บข้อมูลโดยการสัมภาษณ์ ผลการศึกษา พบว่า กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง อายุระหว่าง 30-44 ปี สถานภาพสมรส (คู่) พักอาศัยอยู่ในอำเภอศรีราชา จังหวัดชลบุรี การศึกษระดับมัธยมศึกษา รายได้ต่อเดือนเฉลี่ย 5,001-10,000 บาท รับบริการที่โรงพยาบาลด้วยอาการ อาการแสดงและสิ่งผิดปกติที่พบจากการตรวจทางคลินิก ไม่ได้ไปรับบริการจากที่อื่นก่อนมารับริการในครั้งนี้ และทราบว่าถ้าเลือกโรงพยาบาลสมเด็จพระบรมราชเทวี ณ ศรีราชา เป็นสถานพยาบาลหลักแล้วสามารถไปใช้บริการสถานพยาบาลเครือข่าย ได้รับทราบข้อมูลสถานพยาบาลเครือข่ายจากบริษัท/หน่วยงานที่ทำอยู่ ช่วงเวลาที่กลุ่มตัวอย่างสะดวกไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่ายมากที่สุด คือ ช่วงเวลา 17.00-21.59 น. การวิเคราะห์ความสัมพันธ์ปัจจัยส่วนบุคคล ได้แก่ เพศ อายุ สถานภาพ สมรส ที่พักอาศัย ระดับการศึกษา รายได้ พบว่า ไม่มีความสัมพันธ์กับการไปใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่าย ส่วนปัจจัยด้านการรับรู้ลักษณะการเจ็บป่วย และการประเมินการเจ็บป่วย ปัจจัยด้านการรับรู้สิทธิประโยชน์การใช้บริการทางการแพทย์ และปัจจัยด้านทัศนคติที่มีต่อสถานพยาบาลเครือข่าย อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ระดับคุณภาพการบริการทางการแพทย์ของสถานพยาบาลเครือข่ายโดยรวมทุกด้าน อยู่ในระดับปานกลาง ควรประชาสัมพันธํให้ผู้ประกันตนรับรู้สิทธิประโยชน์ในการใช้บริการทางการแพทย์ที่สถานพยาบาลเครือข่าย ส่งเสริมการรับรู้ ลักษณะการเจ็บป่วย และประเมินการเจ็บป่วยเพื่อเลือกใช้สถานพยาบาลเครือข่ายได้สอดคล้องกับการเจ็บป่วย

The objectives of this study were conducted to study the factors relating to the non-using medical services at network hospitals of insured persons registered at Queen Savang Vadhana Memorial Hospital with the relationship of concerning factors, these factors were related to the personal factor, the perception illness factors and the illness evaluation prior to the medical service acceptation, the perceiving right factors of assertation under the network hospitals, the factor of attitude towards the network hospitals and the medical service quality. The samples of this study were collected from 384 insured persons who have registered as their main contractor and out-patient at Queen Savang Vadhana Memorial Hospital for their illness which were curable only at network hospitals. All data were collected from March to August 2007 and the four sets of questionnaires were used as the research instruments with confidential interval as 0.78, 0.80, 0.76 and 0.82 for interview data collecting. The results showed the majority of samples were female. Age ranged between 30-44 and married status. Their residences was mostly located in Sriracha District, Chonburi Province. Their education backgrounds were found to be high school level and their average income was found to be 5,001-10,000 baths per month. The samples were serviced for illness, symptoms and abnormality which were found from clinical checking; in addition, the samples were not serviced from the other medical centers before receiving the medical service in this time. Moreover, these samples have known that they could receive the medical services from any other network hospitals under the condition of selecting the Queen Savang Vadhana Memorial Hospital as the main contractor. The information of network hospitals was informed from their current working company. The most convenient time for receiving the medical services was found to be 5.00-9.59 p.m. The analysis of relationship between sex, age and marital status, residence, education background and income found to have no significance with non-using medical services at network hospitals. However, the perception illness factors, the illness evaluation, the perceiving right factors of assertion under the network hospitals and the factors of attitude towards network hospitals were found to significantly relate to non-using medical services at 0.05 statistical significant levels. The overall of medical service quality of the network hospitals was found to fair and the suggestions resulted as public participation to the insured persons about their right of assertation under the network hospitals including the promoting about perception illness and the illness evaluation for the selecting network hospitals relating to the illness.
