ผลลัพธ์การบูรณาการการเรียนร่วมกับบริการวิชาการของนักศึกษาเภสัชศาสตร์ชั้นปีที่ 5 มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ โดยการเยี่ยมบ้านผู้ป่วยโรคเบาหวานและความดันโลหิตสูง

Abstract

วัตถุประสงค์ : เพื่อสืบค้นปัญหาการใช้ยาเบาหวานและยาลดความดันโลหิตของผู้ป่วยที่บ้านและรวมทั้งการใช้ยาอื่นที่ไม่เหมาะสมในชุมชน สำรวจจำนวนปัญหาจากการใช้ยาที่ลดลง และความคิดเห็นของผู้ป่วย เจ้าหน้าที่ รพ. สต. และนักศึกษาที่ร่วมกิจกรรม

วิธีศึกษา : ประยุกต์ใช้ความรู้กระบวนการจัดการโรคด้านยาและหลักการบริบาลทางเภสัขกรรม ซึ่งเป็นบริการวิชาการแก่ชุมชน ณ บ้านผู้ป่วย ทุก 2 สัปดาห์ จำนวน 4 ครั้ง วัดผลลัพธ์จากจำนวนปัญหาจากการใช้ยา

ผลการศึกษา : จำนวนผู้ป่วยเข้าร่วมบริการ 100 ราย พบปัญหาจากการใช้ยาในแต่ละสัปดาห์ เท่ากับ 80, 77, 54 และ 36 ปัญหาตามลำดับ ผู้ป่วยที่ได้รับบริการ เจ้าหน้าที่และนักศึกษามีความพึงพอใจอยู่ในระดับสูง

สรุป : ผลการบูรณาการบริการวิชาการกับการเรียนการสอน นอกจากนักศึกษาจะมีความรู้และประสบการณ์แล้ว ยังมีประโยชน์ต่อชุมชนในการส่งเสริมการดูแลสุขภาพและการใช้ยาโดยเฉพาะในกลุ่มผู้ป่วยโรคเรื้อรัง

This objective of integrating education with academic service was to detect and solve drug related problems in diabetic and/or hypertensive patients at home. Method: By applying medication therapy management knowledge and pharmaceutical care concepts into the service, the students were assigned to visit patients at home every 2 weeks for a total of 4 visits under the supervision of their pharmacy professors to assess patients’ drug use. The opinions of health authorities, patients, and students were survey at the end. Result: 100 patients were included in service . By visiting the patients 4 times, 247 issues on drug use were discovered. Providing advices to solve those issues had proven to reduce the number of issues in each visit, from visit 1 to 4:80, 77, 54 and 36 issues, respectively. The opinions of the authorities, patients, and students were mostly satisfied. Conclusion: This project has proven that the integration of education with academic service does not only allow issues on drug use to be discovered and solved, but also provide chances for the pharmacy students to practice their skills in the actual real setting.
