การพัฒนาแนวปฏิบัติการเสริมสร้างพลังอำนาจโดยใช้กระบวนการกลุ่มในการดูแลตนเองด้านการรับประทานอาหารและยาของผู้ป่วยเบาหวาน ชนิดที่ 2 ที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้

Abstract

การศึกษานี้ มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติการพยาบาลในการเสริมสร้างพลังอำนาจ โดยใช้กระบวนการกลุ่มในการดูแลตนเองด้านการรับประทานอาหารและยาของผู้ป่วยเบาหวาน ชนิดที่ 2 ที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ โดยการประยุกต์รูปแบบการปฏิบัติตามหลักฐาน เชิงประจักษ์ของศูนย์ปฏิบัติการพยาบาลขั้นสูงประเทศสหรัฐอเมริกาเป็นหลักโดยผู้วิจัยใช้ 4 ขั้นตอน ดังนี้ 1) ระยะตัวกระตุ้นในการกำหนดปัญหา (Evidence - Trigger) คือผู้ป่วยเบาหวานชนิดที่ 2 ที่มา ใช้บริการที่สถานีอนามัยตำบลบางไผ่ อำเภอเมือง จังหวัดนนทบุรี ที่ไม่สามารถควบคุมระดับน้ำตาลใน เลือดได้ จนทำให้เกิดภาวะแทรกซ้อนตามมา 2) ระยะการทบทวนและสืบค้นหลักฐานเชิงประจักษ์สำหรับ การปฏิบัติที่เป็นเลิศ (Evidence Observed Phase) ด้วยการกำหนดคำสำคัญ และทำการสืบค้นจาก ระบบฐานข้อมูล ห้องสมุดสถาบันวิจัยแห่งชาติ (สวช.)ห้องสมุดสถาบันวิจัยระบบสาธารณสุข (สวรส.) Pub Med National Diabetes Data Group www.Diabassocthai.org และThaiLIS ซึ่งเป็น งานวิจัยเกี่ยวกับการเสริมสร้างพลังอำนาจโดยใช้กระบวนการกลุ่มในการดูแลตนเองด้านการ รับประทานอาหารและยาของผู้ป่วยเบาหวานชนิดที่ 2 ที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ ระหว่างปี พ.ศ. 2541 -2553 และมีระดับความน่าเชื่อถือหลักฐานเชิงประจักษ์ระดับ 1-5 มีทั้งหมด 21 เรื่อง จากนั้นทำการวิเคราะห์และสังเคราะห์งานวิจัย จนได้แนวปฏิบัติการพยาบาลที่ประกอบด้วย 1) แบบสัมภาษณ์ข้อมูลส่วนบุคคลและการตรวจร่างกายเบื้องต้น 2) แบบประเมินความรู้เกี่ยวกับการ ดูแลตนเองด้านการรับประทานอาหารและยาในผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ไม่สามารถควบคุม ระดับน้ำตาลในเลือดได้ 3) แบบประเมินพฤติกรรมการดูแลตนเองด้านการรับประทานอาหารและยา ของผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ และ 4) รูปแบบการ เสริมสร้างพลังอำนาจโดยใช้กระบวนการกลุ่ม ตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 3 ท่าน และตรวจสอบหาค่าความเที่ยงของเครื่องมือ ได้ค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ 0.75 และ 0.85 และ 3) ระยะการนำแนวปฏิบัติการพยาบาลที่พัฒนาขึ้นไปทดลองใช้ (Evidence Observed Phase) ผู้วิจัยได้นำแนวปฏิบัติการพยาบาลที่สร้างขึ้นไปทดลองใช้กับผู้ป่วย จำนวน 30 ราย เป็นเวลา 6 สัปดาห์ พบว่าผู้ป่วยมีค่าเฉลี่ยความรู้ พฤติกรรมการดูแลตนเองด้านการรับประทานอาหาร และยาเพิ่มขึ้น อย่างมีนัยสำคัญทางสถิติ (p<0.05) และ 4) ระยะการวิเคราะห์ ผลของการใช้แนวปฏิบัติ การพยาบาล (Evidence-Based Phase) พบว่าพยาบาลผู้ใช้แนวปฏิบัติการพยาบาลมีความพึงพอใจ เนื่องจาก แนวปฏิบัติที่สร้างขึ้นใช้ง่ายไม่ซับซ้อน

The present study aimed at developing a clinical nursing practice guideline for promotion of empowerment by using a group process for nutrition and medicine care in patients with type 2 diabetes mellitus with uncontrollable blood sugar levels. The evidencebased practice of an advanced nursing care center in the United States of America was employed as the conceptual framework of the study. Four steps used in the development were as follows. First, the evidence-trigger phase involved patients with type 2 diabetes mellitus seeking services at Tambon Bang Phai Health Center, Muang District, Nontaburi Province, were unable to control their blood sugar levels. Second, in the evidence-observed phase, keywords and scope of the search were determined and the search was conducted from the databases of the National Research Institute, the library of the Health System Research Institute, PubMed, National Diabetes Data Group, www.diabassocthia.org, and ThaiLIS. A total of 21 research studies on promotion of empowerment by using a group process for nutrition and medicine care in patients with type 2 diabetes mellitus with uncontrollable blood sugar levels conducted between 1998 and 2010 were retrieved. In terms of credibility of the evidence, all of the 21 selected studies were categorized under levels 1 to 5. After that, the selected studies were analyzed and synthesized. The developed clinical nursing practice guideline were consisted of: 1) the demographic characteristics and initial physical examination questionnaire, 2) the knowledge of nutrition and medicine care in patients with type 2 diabetes mellitus with uncontrollable blood sugar level questionnaire, 3) the nutrition and medicine care behavior in patients with type 2 diabetes mellitus with uncontrollable blood sugar levels questionnaire, and 4) the promotion of empowerment by a group process. Content validity was confirmed by a panel of three experts, and Cronbach's alpha correlation coefficient was employed to determine the reliability of the instruments, which was equal to 0.75 and 0.85, respectively. Third, in the evidence-observed phase, the developed clinical nursing practice guideline was tried out with 30 patients with type 2 diabetes mellitus with uncontrollable blood sugar levels for six weeks. It was found that the patients had higher mean scores of knowledge and nutrition and medicine care with statistical significance (p < 0.05). Finally, in the evidence based phase, the study findings revealed that nurses who implemented the developed clinical nursing practice guideline were satisfied because the guideline was easy to use and uncomplicated.
