ประสบการณ์การดูแลเท้าของทหารที่เป็นเบาหวานชนิดที่ 2 ที่มีภาวะแทรกซ้อนที่เท้า

Abstract

การวิจัยเชิงคุณภาพมีวัตถุประสงค์เพื่อศึกษาประสบการณ์การดูแลเท้าของทหารที่เป็นโรคเบาหวานชนิดที่ 2 และมีภาวะแทรกซ้อนที่เท้า ที่เข้ารับการรักษาในคลินิกเบาหวานโรงพยาบาลค่ายพ่อขุนผาเมือง ผู้เข้าร่วมวิจัยจำนวน 10 คน ได้รับการคัดเลือกแบบเฉพาะเจาะจง ตามเกณฑ์ที่กำหนด ดังนี้ เป็นทหารที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นโรคเบาหวานชนิดที่ 2 และมีภาวะ แทรกซ้อนที่เท้า อย่างน้อย 1 อาการ ได้แก่ ชาเท้า ปวดที่เท้าโดยอาจปวดขณะเดิน และ/หรือ ขณะพัก การรับรู้ความรู้สึกต่าง ๆ ที่เท้าลดลง มีแผลที่เท้า ความผิดปกติของรูปร่างเท้า สามารถสื่อสารด้วยภาษาไทย และยินดีเข้าร่วมการวิจัย เก็บรวบรวม ข้อมูลตั้งแต่วันที่ 19 พฤษภาคม - 30 กันยายน พ.ศ. 2557 โดยการสัมภาษณ์เชิงลึก การสังเกตแบบไม่มีส่วนร่วม การบันทึกภาคสนามและวิเคราะห์ข้อมูลโดยการวิเคราะห์เชิงเนื้อหา ผลการวิจัยจำแนกเป็น 4 ประเด็นหลัก ดังนี้ 1) การรับรู้เมื่อมีความผิดปกติเกิดขึ้นที่เท้า ประกอบด้วย 4 ประเด็นรอง ได้แก่ 1.1) อาการและอาการแสดงของความผิดปกติที่เท้า 1.2) สมรรถนะของการเป็นทหารลดลง 1.3) ขาดความมั่นใจในการดูแล เท้า และ 1.4) รู้สึกเป็นภาระของครอบครัว 2) การดูแลเท้า ประกอบด้วย 5 ประเด็นรอง ได้แก่ 2.1) การป้องกันการเกิดแผลที่เท้า 2.2) การส่งเสริมการไหลเวียนเลือดที่เท้า 2.3) การดูแลรักษาบาดแผลที่เท้า 2.4) การค้นหาความผิดปกติที่เกิดขึ้นกับเท้า และ 2.5) การบรรเทาอาการปวดและชาที่เท้า 3) ปัญหาในการดูแลเท้า ประกอบด้วย 4 ประเด็นรอง ได้แก่ 3.1) ความรับผิดชอบในหน้าที่ 3.2) รองเท้าคอมแบท 3.3) การรักษาเท้าไม่ต่อเนื่อง 3.4) ไม่สามารถตรวจเท้าได้ 4) ความต้องการความช่วยเหลือในการดูแลเท้า ประกอบด้วย 4 ประเด็นรอง ได้แก่ 4.1) รองเท้าคอมแบทที่ตัดพิเศษเฉพาะทหารที่ป่วยเป็นเบาหวาน 4.2) คู่มือการดูแลเท้าสำหรับทหารที่เป็นเบาหวาน 4.3) จัดกิจกรรมการแลกเปลี่ยนเรียนรู้กับผู้มีประสบการณ์การดูแลเท้า และ 4.4) การดูแลต่อเนื่องที่บ้านจากบุคลากรทางการแพทย์

This qualitative research aimed to study the experience of foot care in type 2 diabetes mellitus soldiers with foot complications who were treated at the diabetes mellitus clinic in the Pho Khun Pha Mueang Camp Hospital. Ten participants were recruited by the method of purposive sampling. The criteria of the participants consisted of being diagnosed with type 2 diabetes mellitus and having one of any foot complications: foot numb, foot pain, foot ulcer, and foot deformities, speaking Thai, willing to join in this study. Data were collected from 19 May 2014 to 30 September 2014 by In-depth interviewing, non-participant observation, and field note. Data were analyzed by using content analysis. The results composed of 4 themes. First theme was perceiving when foot abnormal occurred, which consisted of four categories such as signs and symptoms of foot abnormal, decreasingly soldiers competent, lacking in confidence of foot care and feeling burden of the family. Second theme was foot care, which consisted of prevention of foot ulcers, promotion blood circulation at foot, wound care at foot, and searching for any abnormal occurrence at foot, and relieves pain and numbness. Third theme was the problem in foot care, which consisted of responsibility on duty, combat boot, uncontinuous foot treatment, and inability to foot examination. Final theme was the demand in assisting of foot care, which consisted of special combat boot cutting for diabetes mellitus soldiers diabetes foot care manual for the soldiers, activity arrangement on learning and sharing with foot care expert and continuous home care form heath care team.
