การวิเคราะห์ต้นไม้แห่งความล้มเหลว: เทคนิคการชี้บ่งอันตรายเพื่อป้องกันการเกิดอุบัติเหตุจากงาน

Abstract

การสอบสวนอุบัติเหตุเป็นกิจกรรมหนึ่งของการบริหารงานด้านความปลอดภัยเชิงรับซึ่งทำให้ทราบถึงสาเหตุของการเกิดอุบัติเหตุเพียงระดับหนึ่งเท่านั้น การใช้เทคนิคการวิเคราะห์ต้นไม้แห่งความล้มเหลว (fault tree analysis : FTA) เข้ามาช่วย จะทำให้สามารถค้นหาสาเหตุของการเกิดอุบัติเหตุหรือการเกิดความล้มเหลวของระบบได้อย่างละเอียดจนไปถึงสาเหตุพื้นฐาน (basic event) เนื่องจากเป็นเทคนิคในการคิดย้อนกลับที่อาศัยหลักการทางตรรกะวิทยาของ Boolean หาความเป็นเหตุเป็นผลของการเกิดปัญหาหรือความล้มเหลวนั้นโดยประกอบด้วย 4 ขั้นตอน คือ การกำหนดหรือเลือกเหตุการณ์ตั้งต้น (top event) การจัดทำแผนภูมิโครงสร้างต้นไม้ของความล้มเหลว การสรุปชุดสาเหตุ (cut sets) ที่ทำให้เกิดเหตุการณ์ตั้งต้น การจัดลำดับความสำคัญของชุดสาเหตุพื้นฐานเพื่อพิจารณาแก้ไขป้องกันต่อไป การใช้เทคนิคนี้จึงจำเป็นต้องใช้ทีมวิเคราะห์ที่มีความรู้และเข้าใจในรายละเอียดของกระบวนการทั้งหมดที่เกี่ยวข้องกับปัญหาหรือความล้มเหลวนั้น ทั้งนี้การใช้เทคนิค FTA ในการวิเคราะห์เพียงเทคนิคเดียวอาจไม่เพียงพอจึงอาจจำเป็นต้องใจเทคนิคอื่นร่วมเช่น failure modes and effects analysis (FMEA) และ hazard and operability analysis (HAZOP) เป็นต้น

Accident investigation is an activity of advanced safety management which helps us know only some causes of accidents. The use Fault Tree Analysis (FTA) technique could search for causes of accidents or system failures in details and lead us to know basic event. FTA presents failure logic using Boolean logic model to find out rational causes of problems or failures which comprises of 4 processes as follows: setting or selecting top event, constructing Fault Trees diagram, finding the route through a tree between an event and an initiator in the tree called a cut set and setting priority of basic events for considering to solve problems on the next step. By using this technique, the analysis team should have knowledge and well understand in the entire processes relating problems or failures. Only FTA technique, this might not be sufficient, it should combine with other techniques for analysis such as failure modes and effects analysis (FMEA) and hazard and operability analysis (HAZOP) etc.
