ทำการพัฒนาวิธี dot-ELISA สำหรับการตรวจหา IgM ต่อเชื้อเลปโตสไปรา เพื่อใช้ในการวินิจฉัยโรคเลปโตสไปโรซิส โดยเปรียบเทียบกับผลการวินิฉัยทางคลินิก จากตัวอย่างซีรัมจำนวน 130 ตัวอย่าง ประกอบด้วยซีรัมจากผู้ป่วยโรคเลปโตสไปโรซิส ผู้ที่มีสุขภาพดี ผู้ที่อยู่ในพื้นที่ของการระบาดและผู้ป่วยโรคอื่นที่ไม่ใช่เลปโตสไปโรซิส ผลการศึกษาพบว่า ทั้งสองวิธีมีความสอดคล้องกันในระดับปานกลางอย่างมีนัยสำคัญทางสถิติ (Kappa=0.70, P<0.05) ค่าความไว ความจำเพาะ ค่าทำนายผลบวก ค่าทำนายผลลบ และประสิทธิผลชองวิธี IgM dot-ELISA เป็น 74.0, 93.7, 88.1, 85.2 และ 86.1% ตามลำดับ ดังนั้น วิธี IgM-dot ELISA ที่พัฒนาขึ้นจึงสามารถนำไปใช้ในการตรวจกรองผู้ป่วยเลปโตสไปโรซีสในภาคสนามได้ ทั้งยังเป็นวิธีที่สะดวกในการทดสอบและอ่านผล อย่างไรก็ตามการศึกษานี้เป็นเพียงการศึกษานำร่องซึ่งควรจะมีการทดสอบกับกลุ่มตัวอย่างให้หลากหลายมากขึ้น เพื่อสามารถนำไปใช้ประโยชน์ได้สูงสุดในการตรวจวินิจฉัยโรคเลปโตสไปโรซิสทางห้องปฏิบัติการ
Dot-ELISA for detection of IgM leptospiral antibody, was developed and compared with clinical diagonosis. A total number of 130 sera, consisting of patient sera, normal healthy sera, sera of human in endemic area and other diseases sera was collected for the study. Sensitivity, specificity, positive predictive value, negative predictive value and efficacy of IgM dot-ELISA were 74.0, 93.7, 88.1, 85.2 and 86.1% respectively. The agreement rate when the two methods were compared, was 0.51 by kappa analysis. This value demonstrated a moderate agreement. Because of its easier testing, no requirement of experiences and expensive equipments. IgM dot-ELISA can be used for screening of leptospirosis. However, this report was only a preliminary study. Therefore, more sera should be tested before its effectiveness and applicability for serodiagnosis of leptospirosis can be concluded.
