การศึกษาอิสระนี้เป็นการศึกษาวิจัยเชิงคุณภาพที่มีวัตถุประสงค์ เพื่อศึกษาเหตุผล ทัศนคติและแรงจูงใจในการบริจาคเซลล์ต้นกำเนิดของผู้บริจาคโลหิตประจำและปัญหา อุปสรรค พร้อมทั้งเสนอแนวทางในการเพิ่มจำนวนผู้บริจาคเซลล์ต้นกำเนิด จากกลุ่มผู้บริจาคโลหิตประจำ ซึ่งเป็นผู้บริจาคโลหิต 2 ครั้งขึ้นไป เป็นผู้ให้ข้อมูลหลัก 3 กลุ่มได้แก่ กลุ่มที่ 1 กลุ่มผู้บริจาคโลหิตประจำ ที่ยังไม่ได้ลงทะเบียนเป็นผู้บริจาคเซลล์ต้นกำเนิด จำนวน 16 ราย กลุ่มที่ 2 กลุ่มผู้บริจาคโลหิตประจำที่ลงทะเบียนเป็นผู้บริจาคเซลล์ต้นกำเนิดแล้วจำนวน 6 ราย และกลุ่มที่ 3 กลุ่มผู้บริจาคโลหิตประจำที่ลงทะเบียนเป็นผู้บริจาคเซลล์ต้นกำเนิดและได้บริจาคเซลล์ต้นกำเนิดแล้ว จำนวน 2 ราย ทำการรวบรวมข้อมูลโดยใช้วิธีสัมภาษณ์เชิงลึก และการสังเกต หลังจากได้ข้อมูลครบตามประเด็นที่ต้องการจึงทำการวิเคราะห์เนื้อหาด้วยการถอดข้อความจากเครื่องบันทึกเสียงแบบคำต่อคำและทำการวิเคราะห์ข้อมูล ผลการศึกษาพบว่าผู้ให้ข้อมูลหลักเป็นเพศหญิง ร้อยละ 58.33 เพศชาย ร้อยละ 41.67 มีอายุเฉลี่ย 28.92±5.52ปี การศึกษาระดับปริญญาตรีร้อยละ 66.67 และสถานภาพทางครอบครัวร้อยละ 70.83 เป็นโสด ผู้ให้ข้อมูลหลักร้อยละ 33.33 มาบริจาคโลหิต 1 ครั้งต่อปี และร้อยละ 29.17 มาบริจาคโลหิต 2 ครั้งต่อปี เหตุผลที่มาบริจาคโลหิตประจำ ร้อยละ 41.67 มีความเชื่อด้านจิตใจที่อยากช่วยเหลือผู้อื่น ร้อยละ 33.33 มีความเชื่อด้านสุขภาพร่างกายที่ดีขึ้น ผู้ให้ข้อมูลหลักกลุ่มที่ 1 นั้นร้อยละ 31.25 ไม่เคยรู้เรื่องเกี่ยวกับเซลล์ต้นกำเนิดเลย แต่กลุ่มที่ 2 และกลุ่มที่ 3 นั้นรู้เรื่องเกี่ยวกับเซลล์ต้นกำเนิดแล้ว โดยที่ผู้ให้ข้อมูลที่รับรู้แล้วนั้น ร้อยละ 31.58 รู้จากแผ่นพับประชาสัมพันธ์, ร้อยละ 26.31 รู้จากอินเตอร์เนต, ร้อยละ 26.31 รู้จากโทรทัศน์และร้อยละ 10.53 รู้จากเจ้าหน้าที่ประชาสัมพันธ์ ผู้ให้ข้อมูลหลักในกลุ่มที่ 1 ยังมีความรู้และความเข้าใจในเรื่องเซลล์ต้นกำเนิดไม่เพียงพอ แต่กลุ่มที่ 2 และกลุ่มที่ 3 ทราบและเข้าใจข้อมูลเกี่ยวกับเซลล์ต้นกำเนิดและการบริจาคเซลล์ต้นกำเนิดมากกว่าทั้งในเรื่องของประโยชน์ คุณสมบัติรวมถึงวิธีการบริจาคเซลล์ต้นกำเนิด ส่วนเหตุผลที่สนใจทำให้ลงทะเบียนเพื่อบริจาคเซลล์ต้นกำเนิด พบว่าผู้ให้ข้อมูลหลักกลุ่มที่ 1 ร้อยละ 50.00 สนใจเพราะอยากช่วยเหลือผู้อื่น และร้อยละ 37.50 เข้าใจถึงความต้องการใช้เซลล์ต้นกำเนิดในการรักษาผู้ป่วย กลุ่มที่ 2 ร้อยละ 50.00 ลงทะเบียนเพราะมีความรู้เดิมจากอาชีพการงานทำให้มีความเข้าใจ และเห็นถึงประโยชน์ของเซลล์ต้นกำเนิด จึงบริจาคเซลล์ต้นกำเนิด ร้อยละ 33.33 อยากช่วยเหลือผู้อื่น กลุ่มที่ 3 ยินยอมบริจาคเซลล์ต้นกำเนิด เนื่องจากอยากช่วยเหลือผู้อื่นและมีความรู้ในเรื่องเซลล์ต้นกำเนิดเป็นอย่างดี และเหตุผลที่จะทำให้ผู้บริจาคโลหิตไม่สนใจลงทะเบียนเป็นผู้บริจาคเซลล์ต้นกำเนิดนั้น มีเหตุผลมาจากการขาดความรู้ และความเข้าใจ ซึ่งหากผู้บริจาคโลหิตมีความรู้และความเข้าใจเรื่องเกี่ยวกับเซลล์ต้นกำเนิดต้นแล้ว ส่วนใหญ่จะตัดสินใจลงทะเบียนเป็นผู้บริจาคเซลล์ต้นกำเนิด ปัจจัยด้านครอบครัว สังคม และเศรษฐกิจนั้นมีส่วนร่วมในการตัดสินใจ สาเหตุของปัญหาการเพิ่มจำนวนผู้บริจาคเซลล์ต้นกำเนิด พบว่าผู้ให้ข้อมูลหลัก มีความคิดเห็นว่าร้อยละ 37.50 เกิดจากความไม่รู้ ร้อยละ 25.00 เกิดจากความไม่เข้าใจ และร้อยละ 33.33 เกิดจากความรู้สึกกลัว โดยรวมแล้วเกิดจากความไม่รู้ ไม่เข้าใจเรื่องของการบริจาคเซลล์ต้นกำเนิดอย่างเพียงพอ การแก้ไขปัญหาและรณรงค์ เพื่อการเพิ่มจำนวนผู้บริจาคเซลล์ต้นกำเนิด ผู้ให้ข้อมูลหลักทั้ง 3 กลุ่ม ให้ความเห็นไปในทางเดียวกันว่าแนวทางที่ 1 ต้องจัดการด้านการประชาสัมพันธ์ การเพิ่มช่องทางการสื่อประชาสัมพันธ์ รวมถึงการประชาสัมพันธ์ ไปยังส่วนภูมิภาคให้มากขึ้น แนวทางที่ 2 การขอความร่วมมือกับบุคลากรภายในสนับสนุน ในการประชาสัมพันธ์ เชิญชวนผู้บริจาคโลหิตมาบริจาคเซลล์ต้นกำเนิด และขอความร่วมมือจากหน่วยงานในภาครัฐและภาคเอกชน เพื่อตั้งเป็นโครงการหรือชมรมผู้บริจาคเซลล์ต้นกำเนิด เพื่อช่วยส่งเสริมการประชาสัมพันธ์ และแนวทางสุดท้ายการช่วยเหลือด้านงบประมาณและกำลังคน ซึ่งต้องขอการสนับสนุนจากหน่วยงานต่าง ๆ ทั้งภาครัฐและเอกชนให้เข้ามาช่วยเหลือ
This independent study was a qualitative research in order to study the reasons, attitudes, motivations, problems / obstacles, and ways to increase the quantity of bone marrow donors deriving from repeated blood donors who had donated their blood more than 2 times. 24 repeated blood donors were selected as key informants for this study by chance. They were divided into 3 groups:- Group I, 16 repeated blood donors who had not registered to be bone marrow donors yet ; group II, 6 repeated blood donors who had already registered to be bone marrow donors ; and group III, 2 repeated blood donors who had already registered to be bone marrow donors, and donated their bone marrow already. The data were collected by using in-depth interviews and observations. After receiving the complete data according to the raised issues, the data were analyzed by the content analysis methodology. The findings of this study were as followed : 58.33% of key informants were female, their average age was 28.92+5.52 years old, 66.67% graduated at undergraduate level, and 70.83% were single. 33.33% had donated their blood 1 time per year and 29.17% had done 2 times per year. For the reasons of repeated blood donation, 41.67% of them believed that it was good to help other people, 33.33% believed that it was good for their health. 31.25% of group I had not heard about the bone marrow donation before, but group II and III had already known. 31.58% of the ones who had already known about the bone marrow got the knowledge from the brochures, 26.31% from the internet, 26.31% from the television, and 10.53% from public relation (PR) personels, The knowledge and understanding of key informants in group I were poor while the other groups were good including the benefit, the characteristic of donors and the method of bone marrow donation. And the reasons, why they were interested in registering, were found that 50.00% in group I wanted to help other people, and 37.50% had realized the lack of the bone marrow to cure the patients. 50.00% of group II registered because they were health professionals and knew bone marrow, and, thus, they were willing to donate their bone marrow. And 33.33% of them wanted to help other people. Group III, they donated bone marrow because they wanted to help other people and they had known well about the bone marrow. On the contrary, the reasons why the blood donors were not interested in registering to be the bone marrow donors were the lack of knowledge and understanding. If the blood donors had had sufficient knowledge and understanding about the bone marrow, most of them would have registered to be the bone marrow donors. The other factors to make decision in donating their bone marrow were their family, society, and economic situation. The problems/obstacles on the increasing of the quantity of the bone marrow donors were found that 37.50% of the blood donors had insufficient knowledge about the bone marrow, 25.00% had no understanding and 33.33% were discouraging to donate it. The problems were, in general, caused by the insufficient knowledge about the bone marrow. The solution and the campaigns for increasing the quantity of the bone marrow donors were stated from the opinions of all 3 groups in the same way as followed: 1) Increasing the PR achieves, more media and channels of advertising especially to the up country region.2) Cooperating with the donation officers to persuade the blood donors to register as bone marrow donors.3) The budget and manpower supports of both government and private sectors.
