การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม มีวัตถุประสงค์เพื่อพัฒนาแนวทางการเยี่ยมบ้านผู้ป่วยวัณโรคปอดที่รักษาด้วยระบบยาระยะสั้น อำเภอเมือง จังหวัดสมุทรปราการ ภายใต้แนวคิด การจัดการรายกรณีและทฤษฎีการพยาบาลของคิง แบ่งการวิจัยเป็น 3 ขั้นตอน คือ ขั้นที่ 1 ประเมินสถานการณ์การดูแลผู้ป่วยวัณโรคปอด ขั้นที่ 2 พัฒนาแนวทางการเยี่ยมบ้าน และขั้นที่ 3 เปรียบเทียบการรับรู้ของผู้ป่วยก่อนและหลังพัฒนาแนวทางการเยี่ยมบ้าน เก็บข้อมูลด้วยเครื่องมือที่ตรวจสอบความตรงตามเนื้อหา โดยผู้เชี่ยวชาญ 3 ท่าน และค่าความเชื่อมั่นเท่ากับ 0.89 วิเคราะห์ข้อมูลด้วยสถิติพรรณนา Paired t-test และวิเคราะห์เชิงเนื้อหา ผลวิจัยพบว่า กลุ่มตัวอย่างส่วนใหญ่เป็นเพศชาย อายุ 56-65 ปี สถานภาพคู่ จบมัธยมศึกษาปีที่ 1-6 รายได้น้อย คู่สมรสเป็นผู้ดูแล ไม่ออกกำลังกาย ดัชนีมวลกาย < 20 ส่วนน้อยลืมรับประทานยา และเกิดอาการไม่พึงประสงค์จากยา ผลวิเคราะห์บทบาทบุคลากรสุขภาพที่ปฏิบัติขณะเยี่ยมบ้าน พบว่า 2 เดือนแรกติดตามทุกสัปดาห์ 4 เดือนถัดมาติดตามเดือนละครั้ง โดยค้นหาบุคคลทำหน้าที่ พี่เลี้ยง แจกแผ่นพับความรู้ ไม่พบการประสานงานกับผู้นำชุมชน ผลการพัฒนาแนวทางเยี่ยมบ้านได้กิจกรรมที่ระบุบทบาทผู้เกี่ยวข้องที่จำดำเนินงานในสัปดาห์ที่ 1-8 แผนเยี่ยมบ้านที่ระบุวัตถุประสงค์ ขั้นตอนการเยี่ยม และแบบบันทึกการเยี่ยมบ้าน หลังใช้แนวทางเยี่ยมบ้านพบว่าผู้ป่วยรับรู้การรักษาเพิ่มขึ้น (ค่าเฉลี่ยก่อนใช้เท่ากับ 4.11 ค่าเฉลี่ยหลังใช้เท่ากับ 4.75) รับรู้ความเครียดลดลง (ค่าเฉลี่ยก่อนใช้เท่ากับ 2.14 ค่าเฉลี่ยหลังใช้เท่ากับ 1.34) และรับรู้การปฏิบัติของครอบครัว บุคลากรสุขภาพ และชุมชนเพิ่มขึ้นอย่างมีนัยสำคัญ (ค่าเฉลี่ยก่อนใช้เท่ากับ 2.94 ค่าเฉลี่ยหลังใช้เท่ากับ 3.75) การศึกษามีข้อเสนอแนะว่าผู้บริหารหน่วยงานสุขภาพระดับตำบลควรสร้างแรงจูงใจให้เจ้าหน้าที่ นำแนวทางที่พัฒนาขึ้นไปใช้อย่างต่อเนื่อง ประยุกต์ใช้การจัดการรายกรณีและทฤษฎีการพยาบาลของคิงในผู้ป่วยเรื้อรังอื่นๆ วิจัยเปรียบเทียบ 2 กลุ่ม และเพิ่มสื่อที่ทันสมัยในกิจกรรมเยี่ยมบ้าน
This purpose of participatory action research is to develop home visit guideline of pulmonary tuberculosis (TB) for patients treated with short course treatment in Mueang district, Samit Prakan province. The concepts of case management and King's nursing theory were applied. The research is divided into three stages : stage 1 evaluating caring situations of TB of patients. Stage 2 develop guidelines for home visit. Stage 3 : compare the perception of TB patients before and after developing home visit guideline. Data collection tool has been reviewed for content validity by 3 experts and reliability was 0.89. Research tools were composed of questionnaire, in-depth interviews, non-participatory observation and group discussions guidelines. Data were analyzed by descriptive statistics, paired t-test and content analysis. The results showed that most patients were male, aged 56065 years, married, secondary education, less income, spouse as caregiver, physical inactivity, BMI less than 20. Few patients forgot to take medication and had adverse drug reaction. Roles of health personnel during home visit were analyzed and showed that they saw TB patients every 4 months, followed by once a month later. They seek family member to be caretaker, use leaflet to be health media. They did not collaborate with community leader for caring TB patients. The developed guidelines from reflecting and reevaluating roles of stakeholders brought to be 1) guideline which determine roles of the involvements in the 1-8 week of home visit 2) plan which specify the weekly objective and step of home visit 3) the home visit record book. After employ the developed home visit guideline, the perception of TB patients to their treatment was higher (x̄ before use = 4.11, x̄ after use = 4.75), the perception to stress was lower (x̄ before use =2.14, x̄ after use = 1.34), and the perception to family member, health personnel, community leader roles were higher than before using guideline significantly (x̄ before use = 2.94, x̄ after use = 3.95). This study suggested that health administration in community level should support health personnel to use this guidelines continually. Also this guideline should be evaluated periodically. In addition, case management and King's nursing theory shoud be applies with other chornic diseases. Quasi-experimental by comparing 2 groups should be employed and health instruction medias which use modern technologies should induces during home visit.
