ปัจจัยที่ส่งผลต่อการตัดสินใจเข้ารับบริการเทคโนโลยีช่วยการเจริญพันธุ์ในประเทศไทยของคู่สมรสชาวจีน

Abstract

บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์ปัจจัยที่ส่งผลต่อการตัดสินใจเข้ารับบริการเทคโนโลยีช่วยการเจริญ พันธุ์ในประเทศไทยของคู่สมรสชาวจีนในบริบทสังคมและวัฒนธรรม ใช้วิธีวิจัยเชิงคุณภาพ เครื่องมือที่ใช้ในการวิจัย คือแบบสัมภาษณ์เชิงลึกที่สร้างขึ้นตามวัตถุประสงค์ของการวิจัย โดยเก็บรวบรวมข้อมูลจากคู่สมรสชาวจีนและ ผู้เกี่ยวข้องที่สมัครใจให้ข้อมูล จำนวน 30 คน ซึ่งคัดเลือกด้วยวิธีแบบเจาะจงจากโรงพยาบาลเอกชนและศูนย์รักษาผู้มี บุตรยากเอกชนในเขตกรุงเทพมหานคร วิเคราะห์ข้อมูลด้วยการวิเคราะห์เชิงเนื้อหา และนำเสนอผลการศึกษาแบบ พรรณนาวิเคราะห์ ผลการวิจัยพบปัจจัยสำคัญ 4 ด้าน ได้แก่ 1) ปัจจัยส่วนบุคคล คือ ปัญหาสุขภาพด้านการเจริญ พันธุ์และความต้องการมีบุตร 2) ปัจจัยด้านครอบครัว คือ ความกดดันจากผู้ใหญ่ ค่านิยมการสืบสกุล และความ กตัญญูต่อบิดามารดา 3) ปัจจัยด้านบริบททางสังคมจีน คือ ค่านิยมการสืบทอดวงศ์ตระกูล ข้อจำกัดเชิงนโยบายด้าน การเจริญพันธุ์ และค่านิยมของคนรุ่นใหม่ และ 4) ปัจจัยด้านการให้บริการในประเทศไทย คือ ชื่อเสียง ความ เชี่ยวชาญ อัตราความสำเร็จของการรักษา การสนับสนุนเชิงนโยบาย และบริการที่เอื้อต่อผู้ป่วยต่างชาติ ผลการวิจัย สะท้อนให้เห็นว่า การตัดสินใจดังกล่าวมิได้เกิดจากปัจจัยทางการแพทย์เพียงด้านเดียว หากแต่เป็นผลจากปฏิสัมพันธ์ ระหว่างเงื่อนไขส่วนบุคคล ความคาดหวังของครอบครัว ค่านิยมทางสังคมและวัฒนธรรม และโอกาสในการเข้าถึง บริการสุขภาพข้ามพรมแดน ข้อเสนอแนะจากงานวิจัย คือ สถานพยาบาลควรพัฒนาระบบบริการที่คำนึงถึงความแตกต่างทางภาษา วัฒนธรรม และค่านิยมครอบครัวของผู้ป่วยชาวจีน เพื่อส่งเสริมคุณภาพการดูแลผู้ป่วยต่างชาติอย่างเหมาะสม

This article aims to analyze the factors influencing the decision of Chinese couples to receive assisted reproductive technology services in Thailand within socio-cultural contexts. Employing a qualitative research approach, the research instrument used in this study was an in-depth interview guideline developed by the researcher in accordance with the research objectives. Data were collected from 30 key informants who voluntarily participated in the study and were selected through purposive sampling from private hospitals and private fertility centers in Bangkok. The data were analyzed through content analysis and presented in a descriptive-analytical form. The findings revealed four major factors: 1) personal factors, namely reproductive health problems and the desire to have children; 2) family factors, namely pressure from elders, the value of lineage continuation, and filial piety toward parents; 3) factors related to the Chinese social context, namely the value placed on family lineage continuation, policy constraints concerning reproduction, and the values of younger generations; and 4) factors related to services in Thailand, namely reputation, expertise, treatment success rates, policy support, and services that facilitate foreign patients. The findings reflect that the decision of Chinese couples to travel to Thailand to receive assisted reproductive technology services is not determined by medical factors alone. Rather, it results from the interaction among personal conditions, family expectations, social and cultural values, and opportunities to access cross-border health services. The recommendation of this study is that healthcare institutions should develop service systems that take into account linguistic differences, cultural differences, and the family values of Chinese patients in order to appropriately enhance the quality of care for foreign patients.
