ปัจจุบันอุบัติการณ์ความชุกของเชื้อที่ดื้อต่อยาปฏิชีวนะสูงขึ้นกว่าแต่ก่อน และมีแนวโน้มเพิ่มขึ้นทุกปี ขณะที่การรักษาด้วยยาปฏิชีวนะโดยไม่ทราบชนิดของเชื้อ (Empirical treatment) ที่ถูกใช้ในเวชปฏิบัติทั่วไปของการรักษาโรคติดเชื้อในระบบทางเดินปัสสาวะ (UTIs) นั้นมีการใช้มาอย่างยาวนาน จึงเป็นที่มาของคำถามงานวิจัยว่า การให้ยาปฏิชีวนะแบบ empirical treatment นั้นยังมีประสิทธิภาพและเหมาะสมต่อการรักษาโรคทาง UTIs ในผู้ป่วยที่มีโอกาสติดเชื้อดื้อยา (ผู้ป่วยใน) หรือไม่โดยมีวัตถุประสงค์ของงานวิจัย คือ 1) ศึกษาความชุกของเชื้อก่อโรคในทางเดินปัสสาวะของผู้ป่วยในของโรงพยาบาลลาดกระบัง 2) ศึกษายาที่ใช้ในการให้โดยไม่ทราบชนิดของเชื้อ (Empirical treatment) ในการรักษาโรคติดเชื้อทางเดินปัสสาวะของผู้ป่วยในของโรงพยาบาลลาดกระบัง 3) ศึกษาประสิทธิภาพของยาที่ใช้ในการให้โดยไม่ทราบชนิดของเชื้อ (Empirical treatment) ในการรักษาโรคติดเชื้อทางเดินปัสสาวะของผู้ป่วยในของโรงพยาบาลลาดกระบังจำนวน 83 รายโดยความชุกของเชื้อที่เป็นสาเหตุของโรคจะแตกต่างกันไปตามพื้นที่ และยาปฏิชีวนะที่ใช้นั้นเป็น empirical treatment นั้น ไม่ได้มีการเจาะจงให้ใช้ตัวใดตัวหนึ่ง การเลือกใช้จึงขึ้นกับดุลพินิจของแพทย์แต่ละราย และจากการสอบถามทางโรงพยาบาลลาดกระบังพบว่า ยังไม่เคยมีการศึกษาเช่นนี้มาก่อน ทางคณะผู้จัดทำจึงกำหนดการศึกษาในผู้ป่วยในของโรงพยาบาลลาดกระบังขึ้น โดยศึกษาย้อนหลัง (retrospective) จากฐานข้อมูลอิเล็กทรอนิกส์ ระหว่างวันที่ 1 มกราคม – 30 กันยายน 2558 แล้วนำมาวิเคราะห์ข้อมูลแยกตามเพศ โดยใช้สถิติเชิงพรรณนา (Descriptive)ผลวิเคราะห์ความชุกของเชื้อที่แยกจากน้ำปัสสาวะตัวอย่างส่วนใหญ่ให้ผลเป็นลบทั้งในกลุ่มตัวอย่างเพศชาย (34.4%) และเพศหญิง (44.5%) ส่วนตัวอย่างที่ให้ผลบวกพบว่าเชื้อที่พบมากที่สุดคือ เชื้อ E.coli (15.6%) ในเพศชายและ E.coli (ESBL) (25.0%) ในเพศหญิง ยาปฏิชีวนะที่ถูกเลือกใช้มากที่สุดในการเป็น Empirical treatment ในเพศชาย คือ Ceftriaxone (80.0%) รองลงมาคือ Fluoroquinolones (10.0%), Meropenem (6.7%), Augmentin (3.3%) ตามลำดับในเพศหญิง Ceftriaxone (65.9%) รองลงมาคือ Meropenem (1.89%), Fluroquinolones (13.3%), Augmentin (1.9%) ตามลำดับ ด้านประสิทธิภาพในการรักษาด้วย Empirical treatment ตัวอย่างได้รับประโยชน์จากการรักษา 80% ในเพศชาย และ 77.4% ในเพศหญิงการให้ยาปฏิชีวนะแบบ empirical treatment นั้นยังมีประสิทธิภาพในการรักษาและความเหมาะสมกับความชุกของเชื้อที่พบในน้ำปัสสาวะผู้ป่วย โรคติดเชื้อระบบทางเดินปัสสาวะ (UTIs) ในผู้ป่วยใน โรงพยาบาลลาดกระบัง
In the current situation, the prevalence of bacteria that resists to antibiotic more than in the past and has increasing trends every year. While there is treatment with the antibiotic especially of giving Empirical treatment that is treated with the antibiotic while not know type of bacteria, is usually treated UTI for a long time, So that is the question of our research “Are empirical treatments effective and appropriate for treat UTI in the resist to antibiotic patients (in-patients)?The objectives of this research are (1) To study prevalence of the pathogens that cause of UTI in in-patients in Ladkrabang hospital (2) To study the antibiotic that giving empirical treatment to treat UTI in in-patients in Ladkrabang hospital (3) To study the efficiencies of antibiotic that giving empirical treatment to treat UTI in in-patients in Ladkrabang hospital. The prevalence of bacteria that are the cause of UTI is different by living area and the antibiotic that give empirical treatment is not specific, by selection the antibiotics that had to consider about the situation of the drug resistance of that areas and based on discretion of the doctors. Of inquiry from Ladkrabang hospital, they never have to research like this so the sampling group to be used in our research is in-patients in Ladkrabang hospital. To study the retrospective form the electronically information (HosXP) during January 1- September 30, 2015 and analysis the data by gender by descriptive statistics. The results of the prevalence of bacteria that separate from patient’s urine, most is negative result in men is 34.4% and in women is 44.5%. In the other hand, the positive result is E.coli (15.6%) in men and E.coli (ESBL) (25%) in women. The most of antibiotic that giving empirical treatment in men is Ceftriaxone (80.0%), Fluoroquinolones (10.0%), Meropenem (6,7%), Augmentin (3.3%) respectively, in women is Ceftriaxone (65.9%), Meropenem (18.9%), Fluoroquinolones (13.3%), and Augmentin (1.9%) respectively. For the efficiencies of antibiotic that giving empirical treatment in men have benefit result is 80% and 77.4% in women. The giving empirical treatment is still effective for treat UTI and suitable with the situation of the urinary tract infection in in-patients in Ladkrabang hospital.
