งานวิจัยเชิงสำรวจเรื่องผลลัพธ์ของการบริบาลทางเภสัชกรรมในผู้ป่วยที่ได้รับยาวอร์ฟาริน ณ คลินิกผู้ป่วยนอก โรงพยาบาลสมเด็จพระยุพราชสระแก้ว มีวัตถุประสงค์เพื่อศึกษาการเปลี่ยนแปลงของปัญหาจากการใช้ยาวอร์ฟาริน ภาวะเลือดออกก่อนและหลังการให้คำปรึกษาโดยเภสัชกร จำนวนปัจจัยการเกิดปัญหาแต่ละประเภทที่สัมพันธ์กับการเกิดปัญหาการใช้ยาวอร์ฟารินชนิดและจำนวนประเภทของปัจจัยที่สัมพันธ์กับการเกิดภาวะเลือดออก รวมถึง ประเมินความรู้ การใช้ยาวอร์ฟาริน และทัศนคติต่อการได้รับการบริบาล หลังการได้รับคำปรึกษาจากเภสัชกร
โดยทำการศึกษาแบบย้อนหลัง (Retrospective) เก็บข้อมูลด้านปัญหาภาวะเลือดออกปัญหาจากการใช้ยา อันตรกิริยาระหว่างยา โดยใช้แบบบันทึกข้อมูลของผู้ป่วยจากเวชระเบียนผู้ป่วยนอก ที่ทำการรักษาในช่วงปีงบประมาณ พ.ศ. 2558 ตั้งแต่เดือนตุลาคม พ.ศ. 2557 ถึง กันยายน พ.ศ. 2558 และเปรียบเทียบผลลัพธ์ของการบริบาลทางเภสัชกรรมก่อนและหลังการให้คำปรึกษาปัญหาจากการใช้ยาวอร์ฟารินและปัญหาเลือดออก และประเมินความรู้การปฏิบัติตัวและการใช้ยา ทัศนคติหลังจากการได้รับบริบาลทางเภสัชกรรม โดยสัมภาษณ์ผ่านโทรศัพท์ ผลการวิจัยพบว่า ภายหลังการได้รับการปรึกษา โดยเภสัชกร ปัญหาด้านภาวะเลือดออกลดลงร้อยละ 84.0 แตกต่างกันอย่างมีนัยสำคัญ (25 และ 4 ครั้ง , p<0.05) ปัญหาเลือดออกที่พบบ่อยที่สุดคือเลือดออกตามไรฟัน ปัญหาด้านความไม่ร่วมมือในการใช้ยาลดลงร้อยละ 79.1 แตกต่างกันอย่างมีนัยสำคัญ (91 และ 19 ครั้ง, p<0.05) พบว่าการลืมกินยาเป็นปัญหาที่พบบ่อยที่สุด ปัญหาด้านการเกิดอันตรกิริยาระหว่างยาลดลงร้อยละ 96.7 แตกต่างกันอย่างมีนัยสำคัญ (30 และ 1 ครั้ง, p<0.05) พบว่าอันตรกิริยาระหว่างยากับอาหารพบได้บ่อยที่สุด และจากการประเมินผลด้านความรู้ในการใช้ยาพบว่าผู้ป่วยมีความเข้าใจในการใช้ยาอยู่ในระดับปานกลางมากที่สุด คิดเป็นร้อยละ 68.0 ส่วนด้านทัศนคติต่อการได้รับบริการโดยเภสัชกรพบว่าผู้ป่วยทั้งหมด (ร้อยละ 100.0) มีความพึงพอใจต่อกรได้รับบริการระดับมาก
สรุปได้ว่าการบริบาลทางเภสัชกรรมสามารถช่วยลดปัญหาจากการใช้ยาและภาวะเลือดออกได้
Outcomes of pharmaceutical care service among patients using warfarin at outpatient clinic of Somdej Phrayupparaj Sakeaw Hospital province is retrospective cohort study. The objective of this study was to study changing of drug related problems of Warfarin used and bleeding after counseling by pharmacists and quantify each problem factors that related to drug related problems of Warfarin used, type and number of factors related with bleeding, also assess knowledge of Warfarin used and attitude form patient after counseling by pharmacists. Data of which bleeding problems, drug related problem, drug interactions was collected form patient’s data in OPD card during fiscal year 2558, from October 2557 to September 2558. Compare the results of pharmaceutical care before and after consultation of drug related problem of Warfarin and bleeding. And interview over the phone to assess knowledge of Warfarin used and attitude form patient after counseling by pharmacists.
The research found that after counseling by pharmacists bleeding problem decreased 84.0 percent significantly different (25 and 4, p<0.05), the most common symptoms was gum bleeding, noncompliance was significantly decreased (79.1 percent reduction, p<0.05), missing medicines event is the most occurrence. Drug interaction problem was decreased 96.7 percent, significant different (30 and 1, p<0.05) drug-foods interaction was the most occurrence.
The evaluation of knowledge in medicine used, patients had understanding of drug used in moderate scale (68.0 percent). All patients was satisfied by pharmacist counseling.
From the research can be conclude that pharmacist counseling could improve patient’s during related problems of Warfarin used and bleeding problem.
