โรคลมชักเป็นโรคที่เป็นปัญหาสาธารณสุขที่สำคัญ และก่อให้เกิดความพิการตามมาได้ ซึ่งโรคดังกล่าวสามารถรักษาให้หายได้ด้วยยา โดยตัวยากันชักส่วนใหญ่ที่นำมาใช้มักเป็นยาที่มีช่วงการรักษาแคบ (Narrow therapeutic index) การติดตามระดับยาให้อยู่ในช่วงของการรักษาจึงสำคัญเพราะหากระดับยาในเลือดต่ำเกินไปจะไม่สามารถควบคุมอาการชักได้ แต่หากระดับยาในเลือดสูงเกินไปก็จะทำให้เกิดพิษจากยา ซึ่งเภสัชกรจะทำหน้าที่ติดตามระดับยาในเลือด (Therapeutic drug monitoring หรือ TDM )โดยการเจาะวัดระดับยากันชักในเลือดและการคำนวณหาขนาดยาที่เหมาะสม เพื่อให้ผู้ป่วยได้รับความปลอดภัยและประสิทธิภาพสูงสุดวัตถุประสงค์ในการทำโครงงานวิจัยในครั้งนี้คือเพื่อลดอุปสรรคที่ส่งผลต่อการคำนวณขนาดยากันชักที่เหมาะสมให้กับผู้ป่วย โดยขั้นตอนในการสร้างโปรแกรมคำนวณขนาดยาและระดับยากันชักในเลือดนั้นจะใช้ตัวโปรแกรม Microsoft Excel 2013 ซึ่งใช้ข้อมูลทางด้าน Pharmacokinetic ของยากันชักแต่ละตัวโดยผลสรุปจากการประเมินความพึงพอใจของเภสัชกรและนักศึกษาเภสัชศาสตร์ชั้นปีที่ 6 มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ จำนวนทั้งหมด 16 คน หลังจากได้ทดลองใช้โปรแกรม ซึ่งพบว่ามีระดับความพึงพอใจอยู่ในระดับความพึงพอใจมาก โดยเฉพาะในเรื่องของโปรแกรมมีความถูกต้อง แม่นยำในการคำนวณและมีความน่าเชื่อถือของแหล่งข้อมูลอ้างอิง การสร้างโปรแกรมคำนวณขนาดยาและระดับยากันชักในเลือดในครั้งนี้จะช่วยให้เภสัชกรได้รับความสะดวก รวดเร็วและถูกต้องในการคำนวณขนาดยากันชักให้กับผู้ป่วย อีกทั้งยังทำให้ผู้ป่วยได้รับยากันชักในช่วงระดับการรักษาที่เหมาะสมและไม่ทำให้เกิดพิษจากการใช้ยา
Epilepsy is major public health a disease problem that contributes to subsequent disability. The disease could be cure by medication. Therapeutic drug monitoring is necessary because these medicines are narrow therapeutic index .Seizures symptoms will be uncontrollable if drug level in the blood is too low. However, if drug levels in the blood is too high, it will cause drug toxicity. Pharmacist is responsible for therapeutic drug monitoring to monitor drug levels in the blood by measuring levels of epileptic drugs in blood and use a formula to calculate the appropriate dose. This is to ensure that patients receive safe and effective medications.
The objective of this research project is to reduce obstacles that would affect calculation for appropriate level of epileptic drug to patient. Microsoft Excel 2013 will be used to create a calculation of the epileptic drug level in blood. Pharmacokinetic detail of each levels of epileptic drugs and its formulation calculation are applied from Applied Clinical Pharmacokinetic 2008, 2nd edition.
The result of satisfaction by 16 pharmacists and 6th year pharmacy, students of Hua Chiew University, after the trial. It shows satisfaction levels by well especially in calculation and reliability of program and according to the reliable reference source. Creating program to calculate dosage and level of epileptic drug in blood will facilitates the pharmacist to ensure an accurate calculation of epileptic drug level to patients.
