การพัฒนาแนวปฏิบัติการพยาบาลในการส่งเสริมการมารับบริการตรวจเซลล์มะเร็งปากมดลูกของสตรีกลุ่มเสี่ยง : กรณีศึกษาคลินิกเอกชนแห่งหนึ่ง

Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติการพยาบาล ในการส่งเสริมการมารับบริการตรวจเซลล์มะเร็งปากมดลูกของสตรีกลุ่มเสี่ยง โดยใช้โมเดลการปฏิบัติตามหลักฐานเชิงประ จักษ์ของศูนย์ปฏิบัติการพยาบาลขั้นสูงเป็นกรอบแนวคิด โดยการวิเคราะห์สาเหตุจากการทบทวนปัญหาในการปฏิบัติงานและปัญหาจากความรู้ใหม่ๆ ดำเนินการสร้างแนวปฏิบัติโดยการค้นหาหลักฐานเชิงประจักษ์ด้วยการกำหนดคำสำคัญเพื่อสืบค้นข้อมูล ได้งานวิจัยที่เกี่ยวข้องจำนวน 23 เรื่อง จากนั้นนำงานวิจัยที่ได้มาวิเคราะห์และสังเคราะห์ โดยใช้วิธีการวิเคราะห์ทีละเรื่อง แล้วสร้างข้อสรุปในภาพรวมของความรู้ที่ได้จากงานวิจัยร่วมกับการบูรณาการองค์ความรู้จากตำรา และเอก สารที่เกี่ยวข้อง จนได้แนวปฏิบัติการพยาบาลในการส่งเสริมการมารับบริการตรวจเซลล์มะเร็งปากมดลูกของสตรีกลุ่มเสี่ยง ซึ่งประกอบด้วยแนวปฏิบัติการพยาบาล 4 ระยะ คือ ระยะที่1 การคัดกรองและการเชื้อเชิญสตรีกลุ่มเสี่ยง ระยะที่ 2 การให้สุขศึกษา ระยะที่ 3 การเยี่ยมบ้าน ระยะที่ 4 การติดตามและเฝ้าระวังให้มารับบริการตรวจเซลล์มะเร็งปากมดลูกประจำปี แนวปฏิบัติการพยาบาล ได้รับการตรวจสอบโดยผู้ทรงคุณวุฒิจำนวน 3 ท่าน จากนั้นได้นำไปทดลองใช้ในคลินิกโครงการ 4 การพยาบาลและการผดุงครรภ์ ทดลองใช้กับสตรีกลุ่มเสี่ยง เป็นเวลา 6 สัปดาห์ ผลการศึกษาพบว่า แนวปฏิบัติการพยาบาลในการส่งเสริมการมารับบริการตรวจเซลล์ มะเร็งปากมดลูกของสตรีกลุ่มเสี่ยงที่พัฒนาขึ้น สามารถส่งเสริมให้สตรีกลุ่มเสี่ยงมารับบริการตรวจเซลล์ มะเร็งปากมดลูกได้อย่างมีประสิทธิภาพข้อเสนอแนะในการศึกษาครั้งนี้คือควรมีการจัดอบรมให้แก่พยาบาลเรื่องโรคมะเร็งปากมดลูกและฝึกทักษะในการปฏิบัติการพยาบาลเพื่อส่งเสริมการมารับบริการตรวจเซลล์มะเร็งปากมดลูกตลอดจนบูรณาการแนวปฏิบัติการพยาบาลให้เป็นส่วนหนึ่งในการพัฒนาคุณภาพของคลินิกในการส่งเสริมการมารับบริการตรวจเซลล์มะเร็งปากมดลูกของสตรีกลุ่มเสี่ยงอย่างต่อเนื่อง

The purpose of this study was to development the CNPG in terms of pap smear service promotion in a high risk women group. The Center for Advanced Nursing Practice Evidence-based Practice Model (Soukup. 2000) was modified and used as a framework of the study.The development began with problem analysis from actual nursing practice experience and problems from latest knowledges. Keywords were specified in the search of relevant evidences. Twenty-three related literatures were selected. The conclusion from analysis and synthesis of all relevant evidences together with knowledge texts and related literatures were used to determine the CNPG in terms of pap smear service promotion in a high risk women group. As a study, the knowledge that analyses and syntheses from the evidence-based practice, is able to develop a CNPG for serviced promotion of cervical cancer screening in a group of high risk women, which separate in 4 phases. The first phases is screening and invitation. The second phases is to seminar the knowledge of Pap smear and prevention. The third phases is visiting at Home. Lastly is monitoring and taking the eyes on the promotion of women for pap smear check up annually. Furthermore, CNPG was validated by three gynaecologists and experimented at 4th project clinic and obstetrics by trial in 30 high risk women for six weeks. The result is the development of CNPG for the promotion of pap smear service in high risk Women, can use to be a guideline of improving service at health care center for the promotion of pap smear efficiently. The suggestive are the training courses for nurses on the promotion of pap smear service in high risk Women and skill training on how to create awareness among in high risk Women. The CNPG as a part of a continuing quality care development for the promotion of pap smear service in high risk women at clinic unit care.
