การศึกษาเพื่อยืนยันและพัฒนาแบบวัดการปฏิบัติการพยาบาลด้วยหัวใจสำหรับนักศึกษาพยาบาล

Abstract

การวิจัยนี้มีสองระยะ ระยะที่หนึ่งเพื่อศึกษาองค์ประกอบของการปฏิบัติการพยาบาลด้วยหัวใจของนักศึกษาพยาบาลกลุ่มตัวอย่าง ได้แก่ 1) อาจารย์พยาบาล 9 คน 2) นักศึกษาพยาบาลชั้นปีที่ 2 3 4 ชั้นปีละ 3 คน และ 3) ศิษย์เก่า 5 คน รวมทั้งสิ้น 23 คน เก็บรวมรวบข้อมูลด้วยการอภิปรายกลุ่มและสัมภาษณ์รายบุคคล เครื่องมือเป็นข้อคำถามกึ่งโครงสร้าง วิเคราะห์ข้อมูลเชิงคุณภาพ การวิจัยระยะที่สองมีวัตถุประสงค์เพื่อ 1) วิเคราะห์องค์ประกอบของการปฏิบัติการพยาบาลด้วยหัวใจ 2) พัฒนาแบบวัดการปฏิบัติการพยาบาลด้วยหัวใจ และ 3) ศึกษาระดับการปฏิบัติการพยาบาลด้วยหัวใจของนักศึกษา คณะพยาบาลศาสตร์มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ กลุ่มตัวอย่างเป็นนักศึกษาคณะพยาบาลศาสตร์ จำนวน 567 คน เครื่องมือเป็นแบบวัดการปฏิบัติการพยาบาลด้วยหัวใจ มีค่าเชื่อมั่นเท่ากับ 0.96 วิเคราะห์ข้อมูลด้วยการวิเคราะห์องค์ประกอบเชิงสำรวจและเชิงยืนยัน ความถี่ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงมาตรฐาน indepent t - test และ ANOVA ผลการศึกษาพบว่า 1. การปฏิบัติการพยาบาลด้วยหัวใจมี 3 องค์ประกอบ ดังนี้ 1. ความเอื้ออาทร มี 3 องค์ประกอบเชิงชี้วัด ได้แก่ 1) ความเต็มใจให้บริการดุจญาติมิตร 2) การเคารพในศักดิ์ศรีของความเป็นมนุษย์ และ 3) การยอมรับอย่างไม่มีเงื่อนไข 2. การทำงานร่วมกับผู้อื่น มี 2 องค์ประกอบเชิงชี้วัด ได้แก่ 1) มนุษยสัมพันธ์ และ 2) ทักษะการแก้ปัญหา 3. ความรับผิดชอบ มี 3 องค์ประกอบเชิงชี้วัด ได้แก่ 1) การมีเป้าหมายในการปฏิบัติงานและคำนึงถึงผลกระทบต่อผู้อื่น 2) ความศรัทธาในวิชาชีพพยาบาลและ 3) การพัฒนาตนเอง 2. การวิเคราะห์องค์ประกอบเชิงสำรวจและเชิงยืนยัน พบว่า แบบวัดการปฏิบัติการพยาบาลด้วยหัวใจมีจำนวน 48 ข้อมีความสอดคล้องกับข้อมูลเชิงประจักษ์ในระดับที่เหมาะสมอย่างมาก จากการวิเคราะห์องค์ประกอบเชิงยืนยันอันดับสอง ค่าไคสแควร์ (χ2) มีค่าเท่ากับ 23.35 ค่านัยสำคัญทางสถิติ (p) เท่ากับ .07 ดัชนีวัดระดับความสอดคล้อง (GFI) มีค่าเท่ากับ 0.99 ดัชนีวัดระดับความสอดคล้องที่ปรับแก้แล้ว (AGFI) มีค่าเท่ากับ 0.97 ดัชนีความสอดคล้องเปรียบเทียบ (CFI) มีค่าเท่ากับ 0.99ดัชนีรากของกำลังสองเฉลี่ยของเศษ (RMR) มีค่าเท่ากับ 0.01 และดัชนีรากที่สองของความคาดเคลื่อนในการประเมินค่า (RMSEA) (90%) มีค่าเท่ากับ 0.00 3. การวิเคราะห์ระดับการปฏิบัติการพยาบาลด้วยหัวใจ พบว่า นักศึกษาทั้งหมดมีการปฏิบัติการพยาบาลด้วยหัวใจโดยภาพรวมและรายองค์ประกอบอยู่ในระดับปานกลาง ยกเว้นนักศึกษาระดับปริญญาโทชั้นปี 2 มีการปฏิบัติการพยาบาลด้วยหัวใจโดยภาพรวมและรายองค์ประกอบอยู่ในระดับน้อย เมื่อวิเคราะห์เปรียบเทียบการปฏิบัติการพยาบาลด้วยหัวใจจำแนกตามเพศและระดับการศึกษา พบว่า การปฏิบัติการพยาบาลด้วยหัวใจโดยภาพรวมและทุกรายองค์ประกอบมีความแตกต่างกันอย่างไม่มีนัยสำคัญทางสถิติ เมื่อจำแนกตามชั้นปีการศึกษา พบว่า การปฏิบัติการพยาบาลด้วยหัวใจโดยภาพรวมและรายองค์ประกอบ ได้แก่ ความเต็มใจให้บริการดุจญาติมิตร การเคารพในศักดิ์ศรีของความเป็นมนุษย์ มนุษยสัมพันธ์ การมีเป้าหมายในการปฏิบัติงานและคำนึงถึงผลกระทบต่อผู้อื่น และการพัฒนาตนเอง มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 นอกจากนี้พบว่าการยอมรับอย่างไม่มีเงื่อนไขและความศรัทธาในวิชาชีพพยาบาล มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ส่วนทักษะการแก้ปัญหามีความแตกต่างกันอย่างไม่มีนัยสำคัญทางสถิติผลการวิจัยนี้สามารถนำมาใช้เป็นแนวทางในการวางแผนและประเมินผล การจัดกิจกรรม และการศึกษาวิจัยเพื่อการสร้างเสริมอัตลักษณ์ คือ การปฏิบัติการพยาบาลด้วยหัวใจของนักศึกษาพยาบาลได้อย่างเป็นรูปธรรม

This research had been two steps. The first step were purposed to study the definitions and factors of nursing practice by heart for nursing students. Twenty-three participants were consisted of nine instructors, three sephomore nursing students , three junior nursing students and three senior nursing student and five huachiew chalermprakiet university (HCU) nurse alumni. Focus group and indepth interview were used as data collection. Semi-structure interviewed. Data were analyzed by using qualitative analysis. The second step were purposed to 1) study factors of nursing practice by heart, 2) develop a measure of the nursing practice by heart test, and 3) the level of nursing practice by heart for nursing students. The subjects were 567 nursing students of HCU. The instrument was nursing practice by heart test with the confidence at 0.96. Data analysis was per-formed by using confirmatory factor analysis of survey frequency, percentage, mean, standard deviation, indepent t-test and ANOVA. The results of the study found that.1. Nursing practice by heart have three components : 1. Caring indicators have three components 1) willingness to serve as relatives and friends 2) respect for the dignity of the human being, and 3) unconditional recognition. 2. Coorperate indicators have two components 1) interpersonal relationship, and 2) problem solving skills. 3. Accountability indicators have three components 1) performance with goal and concern for any impact on others, 2) faith in the nursing profession, and 3) self-development.2. Exploratory and confirmatory factors analysis found that the measure of nursing practice by heart have 48 items conforming to the empirical data in very appropriate level. Form Second-order factor analysis found that the Chi-square (χ2) 23.35 , statistical significance (p) .07 ,Goodness of Fit Index (GFI) 0.99 , Adjusted Goodness of Fit Index (AGFI) 0.97 , Comparative Fit Index (CFI) 0.99 and Root Mean Squared Residual (RMR) 0.01 and Root Mean Square Error of Approximation (RMSEA) (90%) equal to 0.00 3. Analysis of the nursing practice by heart found that all nursing students had nursing practice by heart overall and individual factors were in the moderate level. The second-year graduate nursing students had nursing practice by heart overall and individual factors composition at was the low level. When classified nursing practice by heart with sex and level of education, it was overall and individual factors included willingness to serve as relatives and friends, respect for the dignity of the human being, human relationship, performance with goal and concern for any impact on others, and self-development were different statistical significance at .01 level. Moreover, the unconditional recognition and faith in the nursing profession were different level of statistical significance at .05 level. The problem-solving skills were not significantly different statistically. This research findings can be used as a guide for activities planning and evaluation. This research is a research for promoting identity of nursing practice by heart for nursing students.
