ระบบบริหารจัดการเพื่อควบคุมโรคเบาหวานและความดันโลหิตสูงโดยพยาบาลอิสระเยี่ยมบ้าน

Abstract

การควบคุมโรคเบาหวาน ความดันโลหิตสูงในประเทศไทยยังไม่ประสบความสำเร็จเท่าที่ควร เนื่องจากมีผู้ป่วยประมาณหนึ่งในสามเท่านั้นที่สามารถควบคุมโรคได้ แพทย์และพยาบาลในระบบของภาครัฐคงมีจำนวนเท่าเดิมหรือลดลงทุกปี การให้บริการผู้ป่วยยังเป็นแบบตั้งรับ บุคลากรไม่มีเวลาเพียงพอในการให้คำปรึกษาแก่ผู้ป่วยเกี่ยวกับการปรับวิถีชีวิตและพฤติกรรมสุขภาพเพื่อควบคุมโรคได้อย่างยั่งยืน การออกแบบระบบบริหารจัดการด้วยลักษณะการจ้างงานพยาบาลอิสระเยี่ยมบ้านให้เป็นพยาบาลประจำตัวผู้ป่วยเบาหวาน ความดันโลหิตสูงแบบเฉพาะเจาะจง การประยุกต์ใช้เครื่องมือช่วยในการตัดสินใจสำหรับพยาบาลอิสระเยี่ยมบ้านในการดูแลควบคุมโรคเบาหวาน ความดันโลหิตสูง และการนำระบบสารสนเทศเข้ามาสนับสนุนการจัดการดูแลผู้ป่วยได้เป็นปัจจุบันช่วยให้พยาบาลปฏิบัติงานตามมาตรฐานอย่างปลอดภัยและมีความทันสมัย พื้นที่เทศบาลนครนนทบุรี มีผู้ป่วยโรคเบาหวาน ความดันโลหิตสูงที่ควบคุมโรคไม่ได้ประมาณร้อยละ 40 ของจำนวนผู้ป่วยเบาหวาน ความดันโลหิตสูงทั้งหมดและมีความพร้อมในการเป็นพื้นที่นำร่องการใช้ระบบบริหารจัดการด้วยลักษณะการจ้างงานพยาบาลอิสระเยี่ยมบ้านเสริมการจัดบริการแบบเดิม

ผลการถอดบทเรียนความสำเร็จของระบบบริหารจัดการและการจ้างงานพยาบาลอิสระเยี่ยมบ้านผู้ป่วยเบาหวาน ความดันโลหิตสูง พบว่า ผู้ป่วยมีระดับน้ำตาลปลายนิ้วและค่าระดับน้ำตาลสะสมในเลือด (HbA1c) ลดลง ค่าเฉลี่ยระดับความดันโลหิตตัวบน (Systolic Blood Pressure) และตัวล่าง (Diastolic Blood Pressure) ลดลง พยาบาลประจำมีภาระงานลดลง พยาบาลอิสระเยี่ยมบ้านมีความมั่นใจและมีความคล่องตัวในการดูแลจัดการปัญหาสุขภาพผู้ป่วยเบาหวาน ความดันโลหิตสูงที่ควบคุมไม่ได้ที่บ้าน

The management of diabetes and hypertension in Thailand has not been as successful as it should be. Approximately about one in three patients can control their condition. The number of doctors and nurses in the public healthcare system remains the same or decreases every year. Diabetes and hypertension care is still reactive, with staff lacking sufficient time to provide counseling on lifestyle modifications and healthy behaviors for sustainable disease management. Implementing a management system by hiring nurse freelances for home visit to care with diabetes and hypertension patients specifically, using decision-making tools for these nurses freelance, and integrating information systems to supportpatient care has helped nurses provide patient care standards, safely, andup-to-date. In Nonthaburi City Municipality, around 40% of diabetes and hypertension patients do not have their control disease, despite the readiness to lead in employing independent nurse freelances for home visit to enhance traditional care services. The results of the management system and the employment of independent nurse freelances for home visit for diabetes and hypertension patients care show a decrease bloodsugar level, as well as a reduction in average upper (SBP) and lower (DBP) blood pressure levels. The workload of community health nurse has decreased, while nurse freelances have gained confidence and flexibility in managing the health problems of diabetes and hypertension patients whose conditions are uncontrolled at home.
