บทบาทพยาบาลเวชปฏิบัติชุมชนกับการดูแลผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยใช้แนวคิดการดูแลที่เน้นครอบครัวเป็นศูนย์กลาง

Abstract

โรคหลอดเลือดสมองเป็นโรคที่พบบ่อยในผู้ป่วยมีอุบัติการณ์และความรุนแรงของโรคเพิ่มมากขึ้น บทความนี้นําเสนอบทบาทของพยาบาลเวชปฏิบัติชุมชนในการดูแลผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยใช้แนวคิดการดูแลแบบครอบครัวเป็นศูนย์กลางในการบูรณาการองค์ความรู้เฉพาะด้านการวางแผนการดูแลสุขภาพเพื่อส่งเสริมพฤติกรรมสุขภาพและลดภาวะแทรกซ้อนในผู้ป่วยที่เป็นโรคหลอดเลือดสมองซึ่งผลลัพธ์ของการดูแลผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยประสานการมีส่วนร่วมของพยาบาลเวชปฏิบัติชุมชนทีมหมอครอบครัวและชุมชน พบว่าในการปฏิบัติพยาบาลแบบครอบครัวเป็นศูนย์กลางของพยาบาลเวชปฏิบัติชุมชน ต้องรับรู้บทบาทตามสรรถนะพยาบาลเวชปฏิบัติชุมชน ในการพัฒนาจัดการและกํากับระบบการดูแลบุคคล กลุ่มคน ครอบครัวและชุมชน การสร้างเสริมสุขภาพการป้องกันโรค การรักษาโรคเบื้องต้น และการฟื้นฟูสภาพโรคด้านความสามารถในการประสานงาน ด้านความสามารถในการเสริมสร้างพลังอํานาจ การสอน การฝึกการเป็นพี่เลี้ยงในการปฏิบัติและด้านความสามารถในการจัดการและประเมินผลลัพธ์ของการดูแลผู้ป่วยโรคหลอดเลือดสมองที่บ้านโดยประสานการมีส่วนร่วมของทีมหมอครอบครัวและชุมชน ดังนั้นการดูแลผู้ป่วยโรคหลอดเลือดสมองที่บ้าน ควรมีการทํางานร่วมกันของพยาบาลเวชปฏิบัติชุมชน ทีมหมอครอบครัวและชุมชน รวมถึงผู้ดูแลผู้ป่วยเพื่อสนับสนุนให้ผู้ป่วยได้รับการฟื้นฟูร่างกาย จิตใจ และการป้องกันภาวะทางสุขภาพที่อาจเกิดขึ้น ซึ่งควรมีการตระหนักถึงคุณภาพชีวิตของผู้ป่วยโรคหลอดเลือดสมอง และผู้ดูแลเป็นส่วนสําคัญที่จะทําให้ผู้ป่วยกลับมามีสุขภาพที่ดีขึ้นได้อย่างรวดเร็ว

Stroke is a common disease among patients, and both the incidence and severity of the disease are increasing. This article presents The Role of Community Nurse Practitioners’ Role in Caring for Stroke Patients at home with an Emphasis on Family - centered Care in Integrating acquired specific knowledge about healthcare planning to promote healthy behaviors and reduce complications for patients with stroke. Outcomes of care for stroke patients at home by coordinating the participation of the family care team and the community, it was found that the family -centered nursing care of nurse practitioners revealed that the perception of the role and competency of nurse practitioners, particularly on aspects of care management, are person group family and community, health promotion health prevention, basic medical care, and rehabilitation, aspect of collaboration, aspect of empowering, educating, coaching, mentoring, and aspect of outcome management and evaluation. There should be a collaboration between care team and the community team, including caregivers to support the patient's physical, and mental rehabilitation and prevention of health conditions that may arise. Quality of life in stroke patients should be taken into account and caregivers it is important for patients to return to better health quickly.
