Abstract: วัตถุประสงค์ วัตถุประสงค์ (หลัก) 1. เพื่อศึกษาความชุกของการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้ (Preventable ADR, pADR) ในโรงพยาบาลตติยภูมิในประเทศไทย 2. เพื่อศึกษาความชุกของการเกิดอาการไม่พึงประสงค์จากการใช้ยา (Adverse drug reactions, ADRs) ในโรงพยาบาลตติยภูมิในประเทศไทย วัตถุประสงค์ (รอง) 1. เพื่อศึกษาปัจจัยที่สัมพันธ์กับการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้ในในโรงพยาบาลตติยภูมิในประเทศไทย 2. เพื่อศึกษาประเภทของยาที่พบได้บ่อยและความรุนแรงของการเกิดอาการไม่พึงประสงค์จากการใช้ยาและการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้ในโรงพยาบาลตติยภูมิในประเทศไทย วิธีการศึกษา การศึกษานี้เป็นการศึกษาแบบย้อนหลัง (retrospective study) โดยอาศัยการทบทวนและวิเคราะห์ข้อมูลย้อนหลังจากผู้ป่วยที่เข้ารับบริการในแผนกผู้ป่วยนอกและผู้ป่วยใน ตั้งแต่วันที่ 1 มกราคม พ.ศ. 2564 ถึง 31 พฤษภาคม พ.ศ. 2568 ณ โรงพยาบาลตติยภูมิในประเทศไทย โดยรวบรวมข้อมูลจากฐานข้อมูลการรายงานอาการไม่พึงประสงค์จากการใช้ยาจากฐานข้อมูลการรายงานอาการไม่พึงประสงค์จากการใช้ยาของเภสัชกร โดยคัดเลือกผู้ป่วยที่พบรายงานการเกิดอาการไม่พึงประสงค์จากการใช้ยาขณะที่ผู้ป่วยเข้ารับการรักษาตัวในโรงพยาบาล เพื่อประเมินความชุกของการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้ ตามแนวทางการประเมินของ Schumock & Thornton และการวิเคราะห์ปัจจัยที่สัมพันธ์กับการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้โดยใช้การวิเคราะห์ความสัมพันธ์ถดถอยโลจิสติกส์หลายตัวแปรแบบขั้นตอน (stepwise multiple logistic regression) ผลการศึกษา การศึกษาครั้งนี้พบผู้ป่วยที่มีอาการไม่พึงประสงค์จากการใช้ยาชนิด active จำนวน 1,029 ราย (1,086 เหตุการณ์) โดยมีความชุกของการเกิดอาการไม่พึงประสงค์จากการใช้ยาชนิด active เท่ากับ 6.90 รายต่อผู้ป่วย 10,000 รายของผู้ป่วยที่เข้ารับการรักษาตัวในโรงพยาบาล พบความชุกของการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้จำนวน 150 ราย (161 เหตุการณ์) คิดเป็น 14.83 รายต่อผู้ป่วยที่มีอาการไม่พึงประสงค์จากการใช้ยา 100 รายของผู้ป่วยที่เกิดอาการไม่พึงประสงค์จากการใช้ยาชนิด active โดยผู้ป่วยที่เกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้มีอายุเฉลี่ย 56.69 ± 24.28 ปี พบว่ามีโรคร่วม ร้อยละ 58.67 และมีการใช้ยาหลายขนาน ร้อยละ 54.00 ซึ่งสาเหตุหลักของการเกิดอาการไม่พึงประสงค์ที่ป้องกันได้เกิดจากการที่ผู้ป่วยมีประวัติการแพ้ยาหรือเคยเกิดปฏิกิริยาต่อยาที่สงสัยมาก่อน ร้อยละ 40.37 รองลงมาคือ ขนาดยา วิถีทาง หรือความถี่ของการบริหารยาไม่เหมาะสมต่อสภาวะของผู้ป่วย ร้อยละ 30.43 และความไม่ร่วมมือในการรักษาของผู้ป่วย ร้อยละ 14.29 กลุ่มยาที่เป็นสาเหตุของการเกิดอาการไม่พึงประสงค์ที่ป้องกันได้มากที่สุด คือ กลุ่มยาต่อมไร้ท่อและเมแทบอลิซึม ร้อยละ 23.92 โดยรายการยาความเสี่ยงสูงที่พบเป็นสาเหตุสูงสุด ได้แก่ warfarin ร้อยละ 33.33 รองลงมาคือ glipizide ร้อยละ 17.55 และ insulin neutral protamine hagedorn (NPH) ร้อยละ 12.00 ตามลำดับ ส่วนอาการไม่พึงประสงค์ที่เกิดขึ้นส่วนใหญ่เป็นปฏิกิริยาประเภท A ซึ่งสัมพันธ์กับฤทธิ์ทางเภสัชวิทยาของยา ปัจจัยที่สัมพันธ์กับการเกิดอาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้อย่างมีนัยสำคัญทางสถิติ ได้แก่ การได้รับยาความเสี่ยงสูง (OR 4.63) ระดับความรุนแรงของอาการไม่พึงประสงค์จากการใช้ยา (OR 2.70) และกลุ่มยาปฏิชีวนะ (OR 0.57) สรุป อาการไม่พึงประสงค์จากการใช้ยาที่ป้องกันได้สามารถพบได้บ่อยในโรงพยาบาล โดยเฉพาะในผู้ป่วยกลุ่มเสี่ยง และส่งผลกระทบต่อความปลอดภัยในการใช้ยาของผู้ป่วย บุคลากรทางการแพทย์จึงควรให้ความสำคัญกับการติดตาม เฝ้าระวัง และป้องกันการเกิดอาการไม่พึงประสงค์จากการใช้ยาอย่างเป็นระบบครอบคลุมทุกช่วงรอยต่อของกระบวนการดูแลรักษาในโรงพยาบาล
Objectives Primary Objectives 1. To determine the prevalence of preventable adverse drug reactions (pADRs) at tertiary care hospital in Thailand 2. To identify factors associated with the occurrence of preventable adverse drug reactions at tertiary care hospital in Thailand 2. To identify commonly implicated drug classes and to assess the severity of adverse drug reactions and preventable adverse drug reactions at tertiary care hospital in Thailand Methods This study used a retrospective design, involving a review and analysis of medical records from patients who received care in both outpatient and inpatient departments at tertiary care hospital between January 1, 2021, and May 31, 2025. Data were obtained from the pharmacist-reported adverse drug reaction reporting database. Patients with documented adverse drug reactions occurring during hospitalization were included to determine the prevalence of preventable adverse drug reactions. The preventability of ADRs was assessed according to the Schumock and Thornton criteria. Factors associated with preventable adverse drug reactions were evaluated using stepwise multivariable logistic regression. Results The study identified 1,029 patients who experienced active adverse drug reactions, comprising a total of 1,086 ADR events. The prevalence of active adverse drug reactions was 6.90 per 10,000 hospitalized patients. Preventable adverse drug reactions were identified in 150 patients, accounting for 161 events, corresponding to a prevalence of 14.83 per 100 patients with active adverse drug reactions. The mean age of patients with preventable adverse drug reactions was 56.69 ± 24.28 years. The mean age of patients with preventable adverse drug reactions was 56.69 ± 24.28 years. Comorbidities were present in 58.67% of patients, and 54.00% were exposed to polypharmacy. The leading cause of preventable adverse drug reactions was a confirmed history of drug allergy or prior reaction to the suspected medication (40.37%). This was followed by inappropriate dosage, route, or frequency of drug administration relative to the patient’s clinical condition (30.43%) and patient non-compliance to therapy (14.29%). Endocrine and metabolic agents were the most frequently implicated drug classes in preventable adverse drug reactions (23.92%). Among high-alert medications, warfarin was the most common causative agent (33.33%), followed by glipizide (17.55%) and neutral protamine Hagedorn (NPH) insulin (12.00%). Most adverse drug reactions were classified as type A reactions, which are predictable and related to the pharmacological effects of the drugs. Multivariable logistic regression analysis showed that exposure to high-alert medications (OR 4.63), greater severity of adverse drug reactions (OR 2.70), and the use of antibacterial agents (OR 0.57) were significantly associated with preventable adverse drug reactions.
