In this qualitative study, the sample included seven patients who have been diagnosed of first stroke disease, seven patients’ family members, and eleven healthcare personnel. Data collection was conducted during February-October 2007. The instrument used was the in-dept interview guidelines participating and non-participating observation, as well as researchers. The validity of the questionnaire instrument was tested in overall parts. The content analysis was performed under the respondent’s right protection. The results have shown that; Firstly, the problems pertaining to family-centered continuous care of patients with first stroke, from the respondent’s viewpoint, regarding to session of planned discharge, the patients’ family obtained inadequate information about readiness and taking care of the patients after leaving the hospital and returned to home. Regarding to session of referring, level of information service transferred to the related work units was low; there was no coordinator during a referring course. Regarding to homecare session, there was a lack of strengthening the family spirit when patient returned to home. Secondly, the needs for family-centered continuing care of patients with first stroke. In discharging session, the individual patient’s family needed for applicable information about readiness in taking care of the patient in practice. They needed for patient’s cooperation in handling things. They needed to adjust the way they thought to accept the realistic incident. In course of referring, forwarding the data was required necessarily; team synergy was needed to take accountability. At home-caring, the assistance among the family members was quite necessary to fulfill the continuing care of the patient with first stroke. Nurse personnel were required to visit the patient’s home for medical advice and recommendation. Thirdly, the expectation in the family-centered continuing care of patients with first stroke, in discharging session, the patient’s family expected that patient’s condition would resume to normality. The family members expected to be taught of physical therapy which would apply to the patient, healthcare instruction by nurses was demanded by patient’s family. In session of referring, the coordinating staffs were expected to transfer related data continuously. In course of home care, the patients and their family expected that nurses would visit them occasionally at home for medical assistance and encouragement. Fourthly, health care practices for family-centered continuing care of the patients with first stroke. In session of discharge, readiness of the family as core caregiver was important. To achieve this, family’s capacity was escalated and participation in healthcare activities in real practice at home was enhanced. Subsequent follow-up was emphasized to encourage caregiver’s capacity in taking care of the patients. In course of referring, data transfer and cooperation have been concentrated, where family file was used a caring-related mechanism. Based on the result of this paper, it’s suggested that family-centered continuous caring for the patients with first stroke should be further improved as useful informative guidelines for first stroke disease in community. Learning how to take care of the patient with first stroke should be improved by building the proactive participation on parties; patients, family, and community. |
