การพัฒนารูปแบบการเรียนรู้แบบสหสาขาวิชาชีพโดยการเยี่ยมบ้านผู้ป่วยโรคเรื้อรัง

Abstract

การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา เพื่อพัฒนาและประเมินผลลัพธ์ของการใช้รูปแบบการเรียนรู้แบบสหสาขาวิชาชีพโดยการเยี่ยมบ้านผู้ป่วยโรคเรื้อรัง ดำเนินการ 4 ระยะ ได้แก่ ระยะที่ 1 การวิเคราะห์สถานการณ์ กลุ่มตัวอย่างเป็นอาจารย์ผู้สอนหลักสูตรด้านวิชาชีพสุขภาพ จำนวน 16 คน ระยะที่ 2 การพัฒนารูปแบบ กลุ่มตัวอย่างประกอบด้วยนักศึกษา จำนวน 90 คน และอาจารย์ที่ปรึกษา จำนวน 8 คน ระยะที่ 3 การนำรูปแบบไปใช้ และประเมินผล กลุ่มตัวอย่างประกอบด้วยนักศึกษา จำนวน 90 คน ผู้ป่วยโรคเรื้อรัง จำนวน 24 คน และอาจารย์ที่ปรึกษา จำนวน 8 คน และระยะที่ 4 การปรับปรุงรูปแบบ เครื่องมือการวิจัยประกอบ ด้วย รูปแบบการเรียนรู้แบบสหสาขาวิชาชีพโดยการเยี่ยมบ้านผู้ป่วยโรคเรื้อรัง แบบประเมินการรับรู้ความสามารถและทักษะในการเรียนรู้กับสหสาขาวิชาชีพ มีความเชื่อมั่น .98 แบบประเมินทัศนคติต่อการจัดกิจกรรมการเรียนรู้แบบสหสาขาวิชาชีพ มีความเชื่อมั่น .83 และแบบบันทึกข้อมูลผู้รับบริการในการเยี่ยมบ้านแบบสหสาขาวิชาชีพ ดำเนินการวิจัยและเก็บรวบรวมข้อมูลในช่วงเดือนเมษายนถึงเดือนมิถุนายน 2562 วิเคราะห์ข้อมูลด้วยวิธีการวิเคราะห์เนื้อหา สถิติ paired t-test และ McNemar’s test

ผลการวิจัยพบว่า 1) การพัฒนารูปแบบการเรียนรู้แบบสหสาขาวิชาชีพโดยการเยี่ยมบ้านผู้ป่วยโรคเรื้อรัง มีกระบวนการสำคัญ คือ การเตรียมความพร้อมของนักศึกษา การเยี่ยมบ้าน และการเรียนรู้โดยการสะท้อนคิด 2) หลังเข้าร่วมการเรียนรู้ นักศึกษามีคะแนนเฉลี่ยการรับรู้ความสามารถและทักษะในการเรียนรู้กับสหสาขาวิชาชีพ และคะแนนเฉลี่ยทัศนคติต่อการจัดกิจกรรมการเรียนรู้แบบสหสาขาวิชาชีพ สูงกว่าก่อนเข้าร่วมการเรียนรู้อย่างมีนัยสำคัญทางสถิติ (p < .001) และ 3) การเยี่ยมบ้านครั้งที่ 3 ผู้ป่วยมีค่าเฉลี่ยความดันซิสโตลิกต่ำกว่าการเยี่ยมบ้านครั้งที่ 1 อย่างมีนัยสำคัญทางสถิติ (p < .05)

จากการวิจัยครั้งนี้มีข้อเสนอแนะว่า บุคลากรทางการศึกษาด้านสุขภาพควรนำรูปแบบการเรียนรู้แบบสหสาขาวิชาชีพนี้ ไปใช้ในการพัฒนานักศึกษาให้มีประสบการณ์ในการทำงานร่วมกับสหสาขาวิชาชีพ เพื่อให้เกิดความร่วมมือที่ดี และทำให้ผลลัพธ์ด้านสุขภาพของผู้รับบริการดีขึ้น

This research and development aimed to develop and examine the effects of the interprofessional education model by home visit in patients with chronic diseases. The four phases study including phase 1: situation analysis, the samples were 16 instructors in courses for healthcare professionals; phase 2: model development, the samples consisted of 90 students and 8 advisors, phase 3: model implementation and examination, the samples consisted of 90 students, 24 patients with chronic diseases, and 8 advisors, and phase 4: model improvement. The research instruments included the interprofessional education model by home visit in patients with chronic diseases, the perceived efficacy and skill in learning assessment form with reliability as .98, the attitude toward interprofessional education assessment form with reliability as .83, and the client’s home visit record form. The implementation and data collection were conducted from April to June, 2019. Data were analyzed by content analysis, paired t-test, and McNemar’s test.

The research results revealed that 1) the development of the interprofessional education model by home visit in patients with chronic diseases focused on the student preparation, the home visits, and the reflective thinking, 2) after the learning participation, the students had statistically significant higher mean scores of perceived efficacy and skill in learning and attitude toward interprofessional education than those of before the learning participation (p < .001), and 3) at the third home visit, the patients with chronic diseases had statistically significant lower mean of systolic blood pressure than that of the first home visit (p < .05).

This research suggests that health educational personnel should apply this interprofessional education model by home visit in patients with chronic diseases for promoting students to have the experiences of practice with interprofessional team in order to enhance a good collaboration and the client’s health outcomes.
