การพัฒนาคลังเวชภัณฑ์ด้วยการจ่ายยา ณ จุดให้บริการ ของโรงพยาบาลส่งเสริมสุขภาพตำบล จังหวัดลพบุรี

Abstract

งานวิจัยเชิงปฏิบัติการนี้มีวัตถุประสงค์เพื่อพัฒนาระบบการบริหารจัดการคลังเวชภัณฑ์ด้วยการจ่ายยา ณ จุดให้บริการ ของโรงพยาบาลส่งเสริมสุขภาพตำบล จังหวัดลพบุรี โดยใช้แนวคิดการพัฒนางานตามวงจรคุณภาพ (PDCA) เป็นการศึกษาในโรงพยาบาลแม่ข่ายจาก 4 อำเภอ และโรงพยาบาลส่งเสริมสุขภาพตำบลในเครือข่าย 55 แห่ง ระหว่างเดือนมีนาคม-กรกฎาคม 2566 โดยการรวบรวมข้อมูลความคิดเห็นจากบุคลากรสาธารณสุขที่รับผิดชอบงานบริหารเวชภัณฑ์ จำนวน 59 คน ผ่านการสำรวจและการสนทนากลุ่มเพื่อวิเคราะห์ปัญหาและวางแผนพัฒนาระบบร่วมกัน ประเมินผลระบบการบริหารจัดการคลังเวชภัณฑ์ใหม่โดยใช้แบบสอบถามและแบบบันทึกตามเกณฑ์คุณภาพและมาตรฐานคลังยาและเวชภัณฑ์ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาโดยเน้นการพัฒนาระบบคลังเวชภัณฑ์และการเบิกจ่ายยาที่มีประสิทธิภาพ ผลการวิจัยแสดงให้เห็นว่า ระบบบริหารคลังเวชภัณฑ์ด้วยการจ่ายยา ณ จุดให้บริการที่พัฒนาขึ้นมาช่วยทำให้มูลค่าการสำรองเวชภัณฑ์โดยรวมลดลงจาก 2,002,170.46 บาท เหลือเพียง 813,433.34 บาทเมื่อเทียบกับก่อนปรับคลังเวชภัณฑ์ มีมูลค่าลดลง 1,188,737.12 บาท หรือคิดเป็นร้อยละ 59.37 และมีอัตราคงคลังเฉลี่ยลดลง 0.69 เดือน หรือร้อยละ 30.80 โดยลดลงจาก 2.24 เดือน เหลืออยู่ที่ 1.55 เดือน แสดงให้เห็นถึงประสิทธิภาพของระบบในการเพิ่มประสิทธิผลของการจัดการคลังเวชภัณฑ์ ลดปริมาณสำรองที่เกินความจำเป็น และเสริมสร้างการใช้ทรัพยากรอย่างมีประสิทธิภาพอีกทั้งยังช่วยลดภาระงานของเจ้าหน้าที่และค่าใช้จ่ายในการบริหารคลังเวชภัณฑ์ ทำให้การบริหารจัดการเวชภัณฑ์มีประสิทธิภาพมากยิ่งขึ้น และสามารถส่งข้อมูลเบิกจ่ายเวชภัณฑ์ได้ทั้งแบบออนไลน์และผ่านโปรแกรมเชื่อมโยงข้อมูล บุคลากรที่เกี่ยวข้องมีความพึงพอใจต่อระบบที่พัฒนําขึ้นอยู่ในระดับมากที่สุด และได้รับการแนะนำให้ขยายการใช้ระบบที่ปรับปรุงขึ้นนี้ไปยังหน่วยบริการอื่น ๆ เพื่อเป็นประโยชน์ในการบริหารจัดการคลังเวชภัณฑ์ของหน่วยบริการสุขภาพต่าง ๆ ต่อไป

This action research aimed to develop a pharmaceutical inventory management system with point-of-service drug dispensing at subdistrict health promotion hospitals in Lopburi Province, using the plan-do-check-act (PDCA) quality improvement cycle. The study was conducted between March and July 2023, involving 4 district hospitals and 55 subdistrict health-promoting hospitals within the network. Data were collected from 59 healthcare personnel responsible for pharmaceutical management through surveys and focus group discussions to analyze problems and cooperatively plan the system development. The new inventory management system was evaluated using questionnaires and records based on quality and pharmaceutical inventory standards. Descriptive statistical analysis highlighted the effectiveness of the developed system. The results revealed that the developed pharmaceutical inventory management system, which consolidates drug dispensing at a single service point, significantly reduced the overall inventory value. The total inventory value decreased from 2,002,170.46 THB to 813,433.34 THB, representing a reduction of 1,188,737.12 THB or 59.37%. Additionally, the average inventory turnover period was reduced from 2.24 months to 1.55 months, a decrease of 0.69 months or 30.80%.These results demonstrate the system's effectiveness in improving inventory management efficiency, minimizing excessive stock levels, and optimizing resource utilization. Additionally, the system reduced staff workload and inventory management costs, significantly enhancing overall efficiency. It also allows both online and program-linked inventory submissions. Staff involved in the system expressed the highest level of satisfaction, and it was recommended that the improved system be expanded to other healthcare units to further enhance inventory management practices.
