ห้องปฏิบัติการเคมีคลินิกมีหน้าที่ตรวจวิเคราะห์ระดับสารชีวเคมีต่าง ๆ ที่มีความสำคัญทางคลินิก เพื่อนำไปใช้ในการวินิจฉัยโรค ซึ่งต้องมีความถูกต้องและแม่นยำ จึงต้องมีการควบคุมคุณภาพทางห้องปฏิบัติการเคมีคลินิก การใช้สารควบคุมคุณภาพชนิดซีรัมแห้ง (lyophilized control serum) เป็นอีกวิธีหนึ่งที่สามารถใช้ในการควบคุมคุณภาพในการตรวจวิเคราะห์ภายในห้องปฏิบัติการเคมีคลินิก การวิจัยครั้งนี้ จึงมีวัตถุประสงค์เพื่อเตรียมสารควบคุมคุณภาพชนิดซีรัมแห้งสำหรับการตรวจวิเคราะห์ระดับน้ำตาลกลูโคสขึ้นใช้เองภายในห้องปฏิบัติการเคมีคลินิก คณะเทคนิคการแพทย์ มหาวิทยาลัยหัวเฉียวเฉลิมพระเกียรติ เพื่อเป็นการประหยัดงบประมาณของห้องปฏิบัติการในการสั่งซื้อสารควบคุมคุณภาพที่มีราคาแพง
งานวิจัยนี้ได้ศึกษาเสถียรภาพของสารควบคุมคุณภาพทั้ง 3 ชนิด คือ ซีรัมเหลวเก็บที่ 4 °C ซีรัมเหลวแช่แข็งเก็บที่ 0 °C และซีรัมแห้งเก็บที่ 0 °C โดยทำการตรวจวัดน้ำตาลกลูโคสในตัวอย่างแต่ละกลุ่ม 2 ระดับ คือ ระดับกลาง และระดับสูง จากการศึกษาพบว่า Optimum Condition Variance (OCV) ได้ค่า %CV ระหว่าง 1.72-3.07 และ Routine Condition Variance (RCV) ได้ค่า %CV ระหว่าง 1.56-2.40 ซึ่งอยู่ในช่วงค่าที่ยอมรับได้ที่กำหนดโดย Barnett ซีรัมเหลวเก็บที่ 4 °C สามารถคงสภาพได้อย่างน้อย 5 สัปดาห์ ส่วนซีรัมเหลวแช่แข็งเก็บที่ 0 °C และซีรัมแห้ง (lyophilized control serum) เก็บที่ 0 °C สามารถคงสภาพได้ อย่างน้อย 3 เดือน เมื่อทดสอบความแตกต่างของการเก็บรักษาสารควบคุมคุณภาพทั้ง 3 ชนิด พบว่ามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<0.05) ซึ่งสารควบคุมคุณภาพชนิดซีรัมแห้งซึ่งมีเสถียรภาพและเหมาะสมใช้เป็นสารควบคุมคุณภาพมากที่สุด
Routine clinical chemistry laboratories are involved in the analysis of biochemical substances that are important and necessary as an auxiliary guide for accurate diagnosis. In order to guarantee the accuracy, precision laboratory results must be incorporated to ensure quality control. Control materials is one parameter which is encountered in quality control. Lyophilized control serum is the control material that is mostly used in clinical chemistry laboratories. The purpose of this project is to produce lyophilized control serum for glucose determination that costs less than commercially available control material.
In this project , three control serum samples stored at different conditions, liquid control serum at 4 °C, frozen control serum at 0 °C and lyophilized control serum at 0 °C, were evaluated for their stability. In this case, two levels (medium and high) of glucose content in each sample category were assessed.
It was found that coefficients of variation (%CV) of optimum condition variance (OCV) and routine condition variance (RCV) were assessed to be 1.72-3.07 and 1.56-2.40, respectively. Both %CV values are accepted by Barnett’s research. In addition, liquid control serum stored at 4°C remained stable for at least five weeks. Meanwhile, liquid control serum and lyophilized control serum stored at 0°C remained stable for at least three months. The stability of all three groups of control serum were significantly different (p<0.05).
In conclusion, lyophilized control serum is the most stable and suitable control material for glucose determination.
