การตรวจวิเคราะห์ระดับ HbA1c มีข้อจำกัดในบางหลักการและเครื่องตรวจวิเคราะห์บางรุ่น เนื่องจากถูกรบกวนโดยฮีโมโกลบินที่ผิดปกติ เช่น ฮีโมโกลบินซี ฮีโมโกลบินดี ฮีโมโกลบินเอส โดยเฉพาะฮีโมโกลบินอี ซึ่งมีอุบัติการณ์สูงในประเทศไทย เพื่อศึกษาผลกระทบของพาหะฮีโมโกลบินอีต่อการตรวจวิเคราะห์ระดับ HbA1c ด้วยหลักการ high performance liquid chromatography (HPLC) และ enzymatic วิเคราะห์ระดับ HbA1c ในตัวอย่างเลือดที่มีชนิดฮีโมโกลบินปกติ (normal hemoglobin typing; A2A) จำนวน 30 ตัวอย่าง และตัวอย่างเลือดที่เป็นพาหะ ฮีโมโกลบินอี (heterozygous hemoglobin E; EA) จำนวน 30 ตัวอย่าง ด้วยหลักการ HPLC (H9HbA1c analyzer) และ enzymatic (A1Care TM analyzer) ระดับ HbA1c ระหว่างกลุ่มตัวอย่างที่มีชนิด ฮีโมโกลบินปกติและพาหะฮีโมโกลบินอี ไม่แตกต่างกันทั้งในหลักการ HPLC (5.88 +- 0.74% และ 5.76 +- 0.48%) และ enzymatic (5.92 +- 0.79% และ 5.83 +- 0.48%) ระดับ HbA1c ที่ได้จากการตรวจด้วยหลักการ HPLC และ enzymatic มีค่าไม่แตกต่างกัน (A2A: 5.88 +- 0.74 % และ 5.922 +- 0.79%; EA: 5.76 +- 0.48% และ 5.83 +- 0.48%) และค่า HbA1c ที่ตรวจได้จากทั้งสองหลักการมีความสัมพันธ์กันอย่างมีนัยสำคัญทางสถิติ (r=0.830, p = 0.000) ระดับ HbA1c ที่วิเคราะห์ได้จากทั้งสองหลักการมีค่าไม่แตกต่างกัน มีความสัมพันธ์กันดี และไม่ถูกรบกวนในตัวอย่างตรวจที่เป็นพาหะฮีโมโกลบินอี
Some principles and instruments of hemoglobinA1c (HbA1c) measurement can be interfered by hemoglobin variants such as hemoglobin C, hemoglobin D, hemoglobin Sand hemoglobin E, which is high incidence in Thailand. This study aims to investigate the effects of heterozygous hemoglobin E on the HbA1c determination by high performance liquid chromatography (HPLC) and enzymatic methods. The HbA1c of 30normal hemoglobin typing (A2A) and 30heterozygous hemoglobin E (EA) samples were measured by HPLC (H9 HbA1c analyzer) and enzymatic methods (A1Care analyzer). The HoA1c levels of normal hemoglobin typing and heterozygous hemoglobin E samples were not different (HPLC: 5.88 +- 0.74% vs5.76 +- 0.48%; enzymatic: 5:92 +- 0.79% vs5.83 +- 0.48%). In addition, the HbA1c levels measured by HPLC were also not significantly different, compared to enzymatic assay, in each hemoglobin typing (A2A: 5.88 +- 0.74%vs5.92+-0.79 %; EA: 5.76 +- 0.48 %vs5.83 +- 0.48%). The statistically significant correlation of the two methods was observed (r = 0.830, p = 0.000). This study suggested that no statistically significant difference in HbA1c levels measured by HPLC and enzymaticmethods. The results analyzed by two assays were positively correlated and the effect of heterozygous hemoglobin E on the HbA1c levels measured by both analyzers was not found.
