การเปลี่ยนถุงมือทางการแพทย์ทุกครั้งตามแนวทางขององค์การอนามัยโลก (WHO) อาจมีข้อจำกัดในการ ปฏิบัติงานภาคสนาม การวิจัยนี้จึงมีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิภาพในการลดปริมาณแบคทีเรียบนถุงมือ ระหว่างการใช้ 75% ไอโซโพรพานอล และ 80% เอทานอล กับการเปลี่ยนถุงมือตามแนวทาง WHO รวมถึงประเมิน ความคงทนของถุงมือทางการแพทย์ตามมาตรฐาน ASTM D5151-19 หลังการทำความสะอาดซ้ำ โดยแบ่งกลุ่ม ตัวอย่างเป็น 3 กลุ่ม ได้แก่ กลุ่มทำความสะอาดถุงมือด้วย 75% ไอโซโพรพานอล (n=51), 80% เอทานอล (n=55) และกลุ่มควบคุมที่เปลี่ยนถุงมือตามแนวทาง WHO (n=18) เก็บตัวอย่างก่อนและหลังดำเนินการด้วยวิธี swab ตรวจ นับจำนวนแบคทีเรียทั้งหมด (Total Bacteria Count) ตามมาตรฐาน BAM และจำแนกชนิดเชื้อด้วยการย้อมสีแกรม และการทดสอบทางชีวเคมี ผลการศึกษาพบว่าการทำความสะอาดถุงมือด้วย 75% ไอโซโพรพานอล และ 80% เอทานอล สามารถลดปริมาณแบคทีเรียได้อย่างมีนัยสำคัญทางสถิติ (p<0.001) โดยลดลงเฉลี่ย 1.63 log10 (97.66%) และ 2.30 log10 (99.50%) ตามลำดับ ขณะที่กลุ่มควบคุมลดลง 1.45 log10 (96.45%) อย่างไรก็ตาม ปริมาณเชื้อคงเหลือในกลุ่มแอลกอฮอล์ทั้งสองชนิดยังสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p<0.01 และ p<0.05) เชื้อที่ พบมากที่สุดคือ Bacillus spp. (45.8%) รองลงมาคือ Acinetobacter spp. (16.7%) และ Staphylococcus epidermidis (12.5%) โดยพบเชื้อก่อโรค เช่น Escherichia coli และ Klebsiella pneumoniae ร่วมด้วย สำหรับการทดสอบความคงทนพบว่าถุงมือไม่เกิดการฉีกขาดหรือรั่วซึม (failure rate 0%) และผ่านเกณฑ์มาตรฐานทั้งหมด สรุปได้ว่าการทำความสะอาดถุงมือด้วยแอลกอฮอล์ โดยเฉพาะ 80% เอทานอล มีประสิทธิภาพสูงในการลดปริมาณแบคทีเรียและไม่กระทบต่อความคงทนของถุงมือ แม้ประสิทธิภาพจะยังด้อยกว่าการเปลี่ยนถุงมือใหม่ตามแนวทาง WHO แต่ผลการศึกษานี้สามารถใช้เป็นข้อมูลสนับสนุนทางเลือกในสถานการณ์ที่ขาดแคลนทรัพยากรหรือการปฏิบัติงานภาคสนามได้ ทั้งนี้ ควรมีการศึกษาทางคลินิกเพิ่มเติมเพื่อยืนยันความปลอดภัยก่อนนำไปใช้จริง
Strict adherence to the World Health Organization (WHO) hand hygiene guidelines, which require changing medical gloves between every patient encounter, often faces operational constraints during field activities or in resource-limited settings. This study aimed to evaluate the efficacy of glove disinfection using 75% isopropanol and 80% ethanol compared with the standard WHO glove-changing protocol in reducing bacterial load, and to assess the structural integrity of the medical gloves after repeated disinfection in accordance with the ASTM D5151-19 standard. Participants were allocated into three groups: the 75% isopropanol disinfection group (n = 51), the 80% ethanol disinfection group (n = 55), and the control group adhering to the WHO protocol (n = 18). Pre- and post-intervention samples were collected using the swab technique. Total bacterial counts were determined using the Bacteriological Analytical Manual (BAM) method, and bacterial species were identified via Gram staining and biochemical testing. Disinfection with both 75% isopropanol and 80% ethanol significantly reduced bacterial contamination (p < 0.001), achieving mean reductions of 1.63 log10 (97.66%) and 2.30 log10 (99.50%), respectively, while the control group showed a reduction of 1.45 log10 (96.45%). However, the residual bacterial loads in both alcohol-treated groups remained significantly higher than that of the control group (p < 0.01 and p < 0.05, respectively). Bacillus spp. was the most frequently isolated taxon (45.8%), followed by Acinetobacter spp. (16.7%) and Staphylococcus epidermidis (12.5%). Notably, hygiene indicators and opportunistic pathogens, including Escherichia coli and Klebsiella pneumoniae, were also detected. Regarding glove durability, no punctures, tears, or water leaks were observed after repeated disinfection (0% failure rate), fully meeting the ASTM standards. Glove disinfection with alcohol-based solutions, particularly 80% ethanol, demonstrates high efficacy in reducing bacterial loads without compromising glove integrity under the experimental conditions. Although its microbial reduction efficacy remains inferior to the standard WHO glove-changing protocol, these findings support glove disinfection as a viable alternative strategy in resource-constrained settings or field operations. Further clinical studies are warranted to validate its safety and effectiveness in real-world practice.
